(415) 577 7400 100 Years May 8, 1986 Report #135 Director, Office of Inspection and Enforcement U.S. Nuclear Regulatory Commission Washington, D.C.

20555

Dear Sir:

On March 10, 1986, Transamerica Delaval notified the Nuclear Regulatory Commission that a potential defect existed with the lube oil sump tank foot valves a component of DSR or DSRV Standby Diesel Generator.

The purpose of this letter is to modify the list of affected sites as a result of our further investigation.

At the time of our March 10 notification, preliminary information was that the valve liner material was deteriorating.

Our final investigation, however, reveals that the valve was being overpressurized causing extrusion of the liner material.

Not all of the sites identified in our March 10 letter are affected by this failure mode.

We have reviewed piping schematics and find the following sites are affected:

UTILITY SITE SERIAL #

MODEL CP & L Shearon Harris 74046-49 DSRV-16-4 TVA Bellefonte 75080-84 DSRV-16-4 WPPSS WPPSS 1 75085-86 DSRV-16-4 Texas Utilities Commanche Peak 76001-04 DSRV-16-4 Consumers Power Midland 77001-04 DSRV-16-4 We are continuing to develop a course for corrective action and will keep all parties appraised of our recommendations.

A copy of this letter will be forwarded to all of the sites affected by our original notification.
