At about 1200 hours on October 19, 1989 Waterford Steam Electric Station Unit.3 was in Mode 6, when an attempt was made to run Containment Fan Cooler (CFC) "C" in slow speed for a containment noise reduction program. The fan motor ran in the reverse direction with reduced air flow and cooling capacity. The CFC "C" fan motor was reworked in October of 1988 following a motor failure and operability of this fan has been assumed since that time; therefore, this event is reportable under 10 CFR 50.73(a)(2)(v). The "C" CFC motor has been rewired for correct slow speed operation and the remaining CFC motors verified for proper rotation. The root cause of this event is personnel error. The required surveillance testing that would have verified proper fan rotation was not performed prior to returning the fan to operabic status. From November 1988 to October 1989 the three remaining CFC's were in service such that the Limiting Condition For Operation of the CFC's was satisfied. Ol 2 l0 - 01 0 d2 0F d4 3 At about 1200 hours on October 19, 1989, with Waterford Steam Electric Station Unit 3 in Mode 6, an attempt was made to start Cont *'.nment Fan Cooler (CFC) "C" fan (EIIS. Identifier.BK-FAN)-in slow speed for noise reduction inside containment

           ~(EIIS Identifier NH). Normal (non-emergency) operation of the CFC fans is fast speed, but due to the reduced heat load during refueling and to eliminate the high noise generated by fast speed operation, a Temporary Alteration Request
           .(TAR) was approved to allow slow speed operation of the CFC during the outage.
           .When the CFC was started, an engineer observed the fan motor rotating in the reverse direction and the gravity damper (EIIS identifier BK-UDMP) closed.

With these conditions, proper air flow and cooling capacity do not exist. Because slow speed operation is a safety function which may not have been available during an emergency, this event is reportable under 10 CFR 50.73(a)(2)(v). A review of the maintenance history for CFC "C" indicated'the following: On May 28, 1988, the CFC "C" supply breaker (Ells Identifier BK-BKR) tripped and

would not reset. Investigation of the problem indicated that all CFC "C" fan motor (EIIS Identifier BK-MO) lead windings (both fast and slow windings) were , shorted to ground. The fan motor was removed and, as a replacement was not available, the motor was shipped off-site to be rewound. On October 24, 1988, with the plant in cold shutdown due to a forced outage, CFC "C" motor was reinstalled. During the period from October 26, 1988, to October 28, 1988, while installation was being completed, several problems were identified with the power cable terminations for the motor. Work authorization (WA) 01019107 documented the individually identified problems and corrections of these problems. Proper fan rotation was checked during the WA implementation by performance of Electrical Maintenance procedure ME-007-006, "480 VAC or less Squirrel Cage Induction Motors." CFC "C" was declared to be operabic on i October 31, 1988. i

0 l2 }0 - 0l0 0 l3 OF 0l4 The root cause of this event is personnel error. Inadequate surveillance testing was performed prior to returning the fan to operability. Surveillance procedure, GP-903-029, " Safety Injection Actuation Signal Test," which would

             . verify system response time and fan DP was not performed. OP-903-029, an 18-month surveillance procedure, had been performed in May 1988 just prior to the motor problem occurrense and was not identified as being required for repeat performance following the motor rework. Surveillance procedure OP-903-094, l'             "ESFAS Subgroup Relay Test - Operating," which was performed to verify the CFC fan would start under simulated emergency conditions does not require verifying proper f an differential pressure (DP) and therefore f an rotation. Contributing to this event was a failure to identify the root cause of the power viring termination problems identified in WA 01019107. When several different wiring problems had to be resolved in completing the WA, supervisors should have determined the cause of these problems, in addition to completing corrective actions.

CFC "C" rotation was corrected under WA 01047636 on October 19, 1989, during Refuci Three. Before startup, all CFC units were tested, per OP-903-029, prior to entering mode 4 where operability is required. To prevent recurrence of this event, the maintenance history on the CFC's will be reviewed and updated to reflect proper retest requirements of the CFC's. This will insure that procedure OP-903-029 is included in future WA's for the CFC's as required. This review and revision will be complete by March 31, 1990. Prior to discovery l l of this event, Waterford 3 has made significant improvements to the " corrective ( actlon program." These improvements included a formal root cause investigation process and training of supervisors in root cause analysis. These changes

l. should preclude the failure to determine " root cause" when significant problems are identified in a WA. Root Cause Investigation (RCI) 89-032 has been initiated to determine the root cause and corrective action for the wiring problems. RCI 89-032 is expected to be completed by January 1, 1990.
