Page3~IV.NATUREOFTHEDEFECTORFAILURETOCOMPLYANDTHESAFETYHAZARDWHICHISCREATEDORCOULDBECREATEDBYSUCHDEFECTORFAILURETOCOMPLY:A350HPmotorwasinstalledonthe"A"SWpumpinJuly,1996.Post-modificationfunctionaltestingrevealedunacceptablemeggerreadings.Themotorwasremovedfromthe"A"SWpump,disassembledandinspected.Theinspectionrevealeddegradationofmotorinsulationintegrityduetodamagedmotorleads,whichcouldresultinthefailureofthemotorinservice.Theoverallqualityofthemotorwasbelowthatnormallyexpectedforacommercialgrademotor.ThesafetyhazardwhichcouldbecreatedbysuchadefectisthecompletelossofServiceWater.Inthescenarioofthe"C"SWpumpoutofserviceandaconcurrentlossofpowertoBus17(whichsuppliespowertothe"B"and"D"SWpumps),alossofallSWwouldoccurifthe"A"SWpumpfailedtooperateduetotheidentifieddefect.V.THEDATEONWHICHTHEINFORMATIONOFSUCHDEFECTORFAILURETOCOMPLYWASOBTAINED:TheinformationwasobtainedonAugust5,1996.VI.INTHECASEOFABASICCOMPONENTWHICHCONTAINSADEFECTORFAILSTOCOMPLY,THENUMBERANDLOCATIONOF'ALLSUCHCOMPONENTSINUSEAT,SUPPLIEDFOR,ORBEINGSUPPLIEDFORGINNASTATION:Therearefour(4)SWpumpslocatedintheScreenhouseBuildingatGinnaStation.A350HPmotorhadpreviouslybeenprocuredseparately.Thismotorwasinstalledtoupgradetheexisting300HPmotoronthe"D"SWpumpinAugust,1995.This350motorisofsimilardesigntothe3motorsdiscussedbelow,andwasalsomanufacturedbyU.S.ElectricalMotors.ThismotorwastestedsatisfactorilyandplacedinserviceinAugust,1995,andoperatedtrouble-free.Asdiscussedbelow,thismotordidnotcontainadefect.Three(3)350HPmotors,manufacturedbyU.S.ElectricalMotors,werepurchasedatalaterdate(asagroup)forupgradeoftheexisting300HPmotorsonthe"A","B",and"C"SWpumps.Asdiscussedbelow,twoofthesethreemotorswerediscoveredtobedefective.

Page4VI~~.THECORRECTIVEACTIONWHICHHASBEEN,ISBEING,ORWILLBETAKEN;THENAMEOFTHEINDIVIDUALORORGANIZATIONRESPONSIBLEFORTHEACTION;ANDTHELENGTHOFTIMETHATHASBEENORWILLBETAKENTOCOMPLETETHEACTION:Thefollowingcorrectiveactionshavebeencompleted:1.GinnaStationMaintenancepersonnelremovedthedefective350HPmotorfromthe"A"SWpump,andshippedthismotorandoneuninstalled350HPmotortoanAppendixBmotorfacility.Thesetwomotorsbothexhibiteddegradationofmotorinsulationintegrity.Themotorsweretested,inspected,andconfirmedtobedefective.Theywererefurbishedbythemotorfacility,andreturnedtoGinnaStation.2.The"A"SWpumpremainedinoperableuntiloneoftheaboverefurbishedmotorswasreinstalledandsatisfactorilytestedinAugust,1996.3.Thesimilar350HPmotor,installedonthe"D"SWpumpinAugust,1995,hadshownnosignsofmotordegradation.However,thismotorwasremovedfromthe"D"SWpumpinOctober,1996,andwasreplacedbytheotherrefurbishedsparemotor.4.Thus,correctiveactionwascompletedforthesetwodefectivemotorsinAugust,1996,andthesetworefurbishedmotorsweresubsequentlyinstalledonthe"A"and"D"SWpumps,andhavefulfilledoperabilitytestrequirements.5~Electricaltestingontheothertwo350HPmotorsdidnotindicateanydegradationofthemotorinsulationintegrity.Nevertheless,asaconservativemeasure,theywereshippedtoanAppendixBmotorfacilityinOctober,1996,fordiagnosticsandtroubleshooting.Theseremainingtwo350HPmotorshavebeendisassembled,inspected,andtestedatanAppendixBmotorfacility.Electricaltesting,performedatthemotorfacility,achievedsatisfactoryperformanceresults,confirmingthatthemotor(previouslyinstalledonthe"D"SWpump)wasoperablewhileinservice.However,someminordeficientconditionswerenotedonbothmotors.Thesemotorswerealsorefurbished.Theserefurbishmentsareconsideredtobeconservativecorrectivemeasureswhichinvolvedmotorreleading,insulationredipandbearingrework.ThefollowingcorrectiveactionsarescheduledforcompletionbyMarch,1997:Safety-relatedmotorCommercialGradeItemEngineeringEvaluations(CGIEE's)willberevisedtoenhancestepsfortheexaminationofwindingsandoverallworkmanshiptoensuremotorsareconstructedwithacceptablequality.CGIEEpreparationguidelineswillbeenhancedtoincorporatethelessonslearnedfromthesedefectivemotors.
