On August 6, 1979, Mr. J. U. Bott of Public Service Company of Indiana, Inc.(PSI) telephoned your office to notify you of a potentially reportable in-cident as required by 10CFR50.55(e) . In November 1978 Co==onwealth-Lord J.V. installed temporary lighting in the Auxiliary Building by utilizing approxi=ately 220 "Hilti" type shot-on studs. These studs were installed into e= bedded safety-related grade A-36 steel plates without an approved procedure or an evaluation as to the potantial effects on the embedded plates.In most cases, these shot-on studs were placed en the edge or in a corner of the embedded plates so as not to interfere with any per=anent fixtures to be welded to the plate. These subject studs are shot onto the embedded plate using a .22 caliber or a .45 caliber shell which forces the approximately 1/8" dia=eter stud shaft into the plate.
