I.EXECUTIVESUMMARYA.ProjectDirectorTheProjectisonscheduleforaFebruary1986fuel1oaddate.Duringthisperiod,theessentialprogressrequiredonthecriticalProjectMilestonesofIntegratedSystemsFlush(whichincludesServiceWaterandCondensateDemineralizerscope)and.DieselGeneratorswassustained.TheProjectachieved1.68%progressfortheperiod,foracummulativepercentcompleteof80.98%.Duringthisperiod,thepercentcompletemethodologywasrevised,effectivethefirstofSeptember.Thechangewaseffectedparticularlyinthemilestonesectionofthecalculation.ThiswasaresultoftheeliminationofServiceWater,CondensateDemineralizerandFlowInducedVibrationmilestonesandtheredistributionofthatscopeintotheremainingmilestones.Inaddition,themethodforcalculatingmilestonecredithasbeenrevisedtoavoidsubjectivityfromtheprocessbyearningcreditwhenaturnovertoStart-upandTestisacceptedorwhenaStart-upandTestReport(STR)isacceptedforpreoperationaltesting.Thevalueforconstructioncommoditieswasreassessedandincreasedby2%.Theplanforprogresstobeearnedisbeingfinalizedandwillbeprovidedinthenextreport.Inthemeantime,wehavecontinuedtocomparetheactualpercentcompletetodateagainstthepreviousplannedprogresscurve.Bulk,installationofcommoditiescontinues,however,threekey'ommodities-cablepulling,largeboresupportsandinstrument~tubing-havefailedtoachievetherateofinstallationplanned.Significantmanagementattentiontoturnthesedeficienciesaroundfsbeingexercised.Improvingthebacklogofcableandcompletingrequiredraceways,increasingJCICraftsmanandredirectionoftheGrinnellworkforcetorequisiteworkandcontinuedhangerimprovementpolicieswillnetpositiveresults.Arevisedcashflowof4675millionfor1984hasbeenapprovedfortheProject.TheProjectfsaggressivelyputsuingconservationmeasuresthatwillresultinloweringtheactualexpendituresbelowthisapprovedlevel.Wearevigorouslyworkingonfinetuningconstructionareasystemschedulesandstart-upandtestschedules.InterfacedisconnectsbetweenConstructionandStart-upanddifferencesbetweenbulkcommodityplansandarea/systemschedulesarebeingresolvedtoensureimprovingtheachievementoftheDieselGeneratorandIntegratedSystemsFlushschedules.The1984INPOEvaluationTeamarrivedonSite9/24/84asplannedandwillbeconductingtheirauditfrom9/24/84toll/5/84toevaluatecontrolsimplementedfordesign,designchanges,constructionmethods,qualityinspections,relatedtrainingandthedocumentationprocesses.ThefullsupportoftheProjecthasbeenprovidedtotheINPOTeamtoassistinameaningfulcomprehensiveauditwithfruftfullresultsanticipated.DuringSeptember,theProjecttransferreditsemployeeexitinterviewprogramtothenewlyestablishedcorporate-widegualityFirstProgram.

MAJORPROBLEMS~~Ourabilitytocontrolworkwithinschedulerequiressignificantimprovement."Improvementshavebeenslowtodate,withsignificantworkremainingtobedone.ThisproblemisamajorsubjectoftheProjectImprovementProgram.Inaddition,managementisdedicatingitsdirectattentiontoturningthisproblemaround.Cablebacklogrequiredtosupportplannedcablepullingratesneedstobeimproved.Althoughthebackloghasshownimprovementrecently,itisstillnotatasatisfactorylevel.Inaddition;whileconduitinstallationprogresseswellinabulkmode,it'isnotbeingworkedadequatelytomaximizecablepullingactivities.AtaskforceofProjectpersonnelhasbeenorganizedtoachieveagoalofthreeweeksbacklogtosustaincablepullingbytheendofOctober."Precisedefinitionofcomponentswithintestboundaries,"whilestillaconcern,isimproving.Projectorganizationscontinuetoworktoimprovethisprocess."TheRecordsIdentificationandManagementProgrammustbebroughtwithintotalcontrolattheearliestdate."Thisiternhasdemonstratedsatisfactoryimprovementandwillnolongerbediscussedhereasamajorproblem."InanumberofsignificantareaswehavenotbeensatisfiedwithGeneral,...ElectricCompany'sattentiontoProjectschedulerequirementsunderthe."NSSSscopeofwork."ChangesinGeneralElectricmanagementand".:continuedGeneralElectricServiceManagementattentionpermitsthe"removalofthisitemhereasamajorproblem."ITTGrinnellhasnotbeenabletomeettherecoveryplanforLargeBoreCategoryIPipeSupports.Additionally,thelargeborehangerrejectrateisunacceptable."Asstatedpreviously,theredirectionoftheITTGrinnel1workforcetorequisftewotk,coupledwiththehangerimprovementprogram,shouldresultinacceleratingtherecoveryplan.TheQPMPReviewBoardhasrecentlyapprovedspecificactionsthatareexpectedtoresultinimprovingthehangeracceptancerate."As-bui1tProgramhasfallenbehindschedule."AdetailedscheduleconsistentwiththeIntegratedSystemsFlushMilestoneisunderdevelopment.ITT-Ghasbeenadvisedtoredirecttheirworkforcetothisarea.

I.EXECUTIVESENARYB.ManaerQualityAssurance-ProjectsTheoverallqualityofon-goinghardwareinstallationsappearstobesatisfactory.Deficienciesthatareidentifiedaredocumentedandsatisfactorilyresolvedwithinthevariouscontractor'sQualityAssurancePrograms.However,enhancementsarecontinuingtobemadeinthecontractor'sprogramstoassurethatadequatesoftwareisavailabletosubstantiatetheinstallationactivities.Inordertoacceleratetheseenhancements,NMPC-QAisperforminganassessmentofeachoftheContractor'sQualityAssurancePrograms.Theseassessmentactivitiesarediscussedbelow.DuringareviewofsomeofthemajorcausesofQCrejectionofITTwelds,itwaslearnedthatITT'sprocedurerequiredthateachwelderstencilhisweld,whileSWEC'sproceduredidnot.Coderequirementsweremetbybothprocedures(ITTandSWEC)byhavingweldandwelderidentifiedonawelddatasheet.ThisdifferenceinprocedurewasremovedinAugust1984,therebyeliminatingoneofthemajorcausesofthehighrejectrate.However,sincetherateofrejectsstillhasnotreachedanacceptablelevel,correctiveactionswillcontinue.Stone5Webster'sdetailedauditoftheSWECQualityAssuranceMonitoringProgramforcontractorshasbeencompletedandthefinal.-reporthasbeenissued.Severalauditobservationsrequirea.'esponsefromSWECQA.Theobservationsaddressenhancementsto;SWEC'sQAmonitoringandrecordsreviewprogram.NNPCProjectQualityAssuranceperformedadetailedassessmentoftheeffectivenessandadequacyoftheon-sitecontractorsprograms.Todate,theassessmentsofJohnsonControlsInc.(JCI),ReactorControlsInc.(RCI),ITTGrinnell,andSWEChavebeencompleted.Afinalreportwillbeissuedsumaarizingtheresults.Todate,oneCorrectiveActionRequestandfiverecommendationshavebeenidentifiedforJCI.TenCorrectiveActionRequestsand14recommendationshavebeenidentifiedforRCI,alladdressingsoftwareproblems.ThefinalreportforITTGrinnellandSWECispresentlybeingevaluatedtodeterminetherequiredcorrectiveactionsandrecommendations'eeded.TheIndependentAssessmentTeamiscontinuingitsreviewofcompletedCATActionPlans,NRCdeficienciesidentifiedintheSALPReport,andNHPC/Contractoridentifieddeficiencies.
