Contrary to-the above, activities affecting quality were not accomplished in accordance with prescribed drawings in that:

1. On March 26, 1981 rebar spacing at the reactor coolant pump base blockouts was zero to 3/4-inch in three places.
2. On' April 15, 1981 rebar spacing at the pressurizer shield wall was '

zero to one-inch in one area.

Response

1. Dake admits violation.

l

2. Craft and inspection personnel either failed to detect the rebar spacing problem or did not obtain proper written authorization to allow closer spacing of the bars.
3. As a result of an earlier NRC Inspection on 1/25/80 a similar problem with rebar spacing was found and subsequent training was conducted on 1/29/80 in rebar placing tolerances per ACI-301-72 and design specifi-cation CNS 1109.00-1. Also, as part of the training, inspectors were instructed to check rebar dowels after concrete placement for proper

' spacing and cover. The concrete pours which this violation affects were 4

made prior to the above described training: RBf2 Reactor Coolant Pump

! Support block-out near 220 degrees - part of pour 2140 placed on 6/19/78

' and RB#2 Pressurizer Wall Dowels in operating floor pour 4200 placed on 10/11/79.

Nonconforming Item #'s 11525 and 11667 were generated as a result of this violation to report the spacing problems which were evaluated and found acceptable as are.

'4. Corrective action per NCI's 11525 and 11667 requires additional training of inspectors and craftsmen in rebar placing tolerances.

5. Applicable training has been performed and no further corrective action is deemed necessary.
