Dear Mr.Murley:

Rochester Gas and Electric is submitting the attached 10CFR Part 21 Evaluation and Notification.

The notification concerns a defect in design of two (2)Residual Heat Removal Pumps supplied by Pacific Pump, Inc.of Los Angeles, CA.The defect consist of an interference between the mechanical seal pumping ring and the pump discharge housing.The defective designed pumping rings were installed in 1969, during the construction phase, as a result of a mechanical seal modi fi cati on.The mechanical seal was suppli ed by Crane Packing Company.Due to the date of occurrence, the condition is technically not 10CFR Part 21 reportable.

However, due t'o the significance of'he potential pump degradation, Rochest'er Gas and Electric is submit'ting voluntary notification.

Specific information regarding fai lure i dent'ifi cation and corrective act'i on taken are detailed on the enclosed Att'achment'.

Supplier: Sub-Suppli er: Nature of Defect: RHR Pumps Pacific Pump Inc., Los Angeles, CA Crane Packing Company Mechanical seal pump ring interference with pump discharge housing.In discussion wi th Original Equipment Manufacturer, discovered during a root cause i nvesti gati on, there was a di mensi onal tolerance issue between the originally supplied mechanical seal outside diameter and the pump casing bore inside diameter.It was determined the mechanical seals were changed out during the plant construction phase and replaced with an alternate mechanical seal.Apparently, at this time, the original pumping ring was replaced with a new style ring with no attention paid to the impact of this change with respect to pump casing bore inside diameter.Date obtained: Rubbing between the pumping ring and the pump housing could cause degradation to the pumping ring with the potential for damage to the pump rotating assembly.Condition is technically not Part 22 violation due to date of occurrence (prior to 2978).April 28, 2989 Basic Components Affected: Number: Locati on: 2 Auxiliary Building, Sub-basement Corrective Action: Machined outside diameter of pumping ring at supplier<s direction to permit proper placement in pump discharge housing.

Responsible Individual for Action: Thomas A.Marlow, Superintendent Ginna Support Services Time to Complete Action: Action was completed on both pumps Apri l 29, 1989.Advice Related to Defect': Vendor supplied repair direction.
