PRELIMINAR_Y H_0TIFI_ CATI _0N OF EVENT OR UNUSUAL OCCURRENCE--PNO-III-86-132 Date November 10, 1986 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by the Region III staff on this date.

10 OPERATORS FAIL WRITTEN EXAM Ten of 24 reactor and senior reactor operators failed the written portion of the NRC's requalification examination. One of the 10 also failed the oral test.

The examinations were administered by NRC examiners the weeks of Septen,ber 15 and October 13, 1986, at Quad Cities Units 1 and 2.

The results indicate weaknesses in the operators knowledge of normal, abnormal and emergency operating procedures, and plant technical specifications.

A Confirmatory Action Letter will be issued by Region III on November 10, 1986. The letter will confirm the licensee's agreen.ent to staff each shift with at least one licensed Senior Reactor Operator who has recently passed an NRC administered exam. These SR0s will act as advisors. The licensee will develop and administer an accelerated program to requalify licensed personnel, and develop a long-term improvement program to preclude reoccurrence of similar deficiencies.

Region III (Chicago) will meet with the licensee November 13, 1986, to review the licensee's upgraded program plan and the implen,entation schedule.

Unit I was taken to cold shutdown on November 7,1986, for a previously scheduled maintenance outage. Unit 2 is in a refueling outage.

The State of Illinois will be notified.
