NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos. 50-254; 50-265 Quad Cities Station, Units 1 and 2 License Nos. DPR-29; DPR-30 During an NRC inspection completed on October 14,1997, to April 15,1998, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and .

Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:

l 10 CFR 50, Appendix B, Criterion ll, " Quality Assurance Program," required, in part, that the licensee's quality assurance program shall provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and maintained.

Contrary to the above, A. On December 15,1997, the inspector identified that the licensee failed to provide training of personnel performing activities affecting quality as necessary to assure that

suitable proficiency was achieved and maintained. Specifically, the inspector identified one individual assigned in the Unit 1 battery charger room to implement compensatory actions, who could not readily identify the correct breakers as delineated in Quad Cities Annunciator Procedure (QCOA) 0010-12. " Fire / Explosion," Revision 7, Attachment D.

I B. On December 9,13-16,1997, the licensee failed to provide training of personnel performing activities affecting quality as necessary to assure that suitable proficiency was achieved and maintained. Specifically, the licensee identified that three individuals did not pass the written examinations prior to being assigned to perform compensatory actions in the plant. These examinations were administered to ascertain that personnel, j who would operate safety-related breakers in an event of a fire, had suitable proficiency i

to perform such activities.

!. ' actions taken and planned to correct the violation and prevent recurrence and the date when l
