PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-77-195 This areliminary notification constitutes EARLY nbtice of an event of POSSI 3LE safety or public interest significance.
The infonnation presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

LOSS OF POWER TO QUAD-CITIES UNIT 2
! CoEnonwealth Edison Company notified RIII (Chicago) that at 7:29 a.m.
.on November 6,1977, a transformer, that feeds power to the canal spray
~,
l pumps, faulted. causing a loss of nonnal offsite power':for Unit 2 and a reactor trip, During the transient, Quad-Cities Unit I also trippel for as yet unknown reasons.
The shutdown of Unit 1 was normal with all emergency core. cooling system (ECCS) equipment working i
as required. The Unit 2 ECCS equipment also performed as required with i
the exception of a faulty relief valve which caused the Unit to blowdown twice.
l The first blowdown depressurized the reactor from-1100 psig to 340 psig j
-over a period of 20 minutes, before the valve closed. - Approximately 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />

1ater when the pressure had increased to 1050 psig due to decay heat, the operator manually actuated the relief valve for pressure control.
l The pressure again decreased papidly from 1050, psig to 140 psig over a period of I hour. The depressurization..then. continued at a slower rate 4
indicating that the valve was still partially open. '
The licensee intends to perform an engineering evaluation of the cooldown" effects on the primary system and determine the caose of the faulty relief valve operation before resuming operations.
No offsite releases occurred due to this incident.
The State of Illinois and State of Iowa are being notified.
RIII (Chicago) is considering issuing a press release.
An inspector h'as been dispatched to the site.
RIII (Chicago) received notification of this occurrence on November 6, 1977, at 11:00 a.m. via telephone from the Plant Superintendent.
