POSSIBLE TAMPERING (FIRST UPDATE)

The licensee has completed its investigation of the possible tampering incident involving the overcurrent relay for the Containment Spray Pump 18 and concluded that tampering was unlikely. The investigation determined there was a plausible explanation for the circumstances surrounding the incorrect overcurrent setpoint.

(The pump failed to start July 29, 1985, and the licensee determined that the setpoint for the overspeed relay was 19.6 amps instead of the normal 40-amp setting.)

The licensee determined that the lead wire seal had been improperly installed (rather than tampered with) and that the damage to the plastic cover of the relay box had been previously reported with a pending work request for its repair.

The improper setpoint was attributed by the licensee as an apparent personnel error which occurred in 1984 when more than 200 relays were set. The pump start range is 10.95 to 21.9 amps, and so the erroneous setpoint could have existed since 1984 with no pump failures.

The Senior Resident Inspector and a region-based security inspector have reviewed the c licensee's investigation and findings with the conclusion that the investigation was thorough and the conclusions appropriate. No further action is planned by Region III (Chicago).

This information will be provided to the State of Missouri.
