The information is as initially received without veri-fication or evaluation, and is basically all that is known by the staff on this date.

CAL ISSUED BECAUSE OF FAULTY INSTALLATION On July 19, 1985, Region III issued a Confirmatory Action Letter requiring the licensee to

< review all cancelled or rejected drawing change requests (DCRs) on Units 1 and 2 to verify th3 status of the DCRs prior to restarting both reactors. (The units are currently in cold shutdown.)

The CAL was issued as a result of a July 17, 1985, report in which the licensee said four Residual Heat Removal (RHR) shutdown cooling suction high flow isolation switches in Unit I had been installed properly. The licensee discovered the problem while performing a special test to confirm the operability of ECCS instrumentation.

The licensee determined that the installation error was caused when a change in piping to the switches in 1982 was not reflected in the field drawing. As a result, a later modification to the switches was based on incomplete information. Piping to the switches was installed backwards, and the switches would not have worked if called upon.

The safety significance of this event is minimal since the system is isolated during reactor operation, startup or hot standby.

Unit I has been operating since March 1985 with the erroneous installation.

The State of Illinois will be notified.
