Joseph M. Farley Nuclear Plant, Unit 2 Special Report 2011-001-00, Inoperable Radiation Monitor R-29B Enclosure Special Report 2011-001-00 This Special Report is being submitted due to the Unit 2 radiation monitor R-29B (Plant Vent Stack) not being restored to operable status within the time required by Technical Requirements Manual (TRM) TR 13.3.4. The TRM requires the monitor to be restored to operable status within 7 days or submit a Special Report within the next 14 days following the event.

The Required Action Statement was entered at 01: 18 on November 5, 2011 when radiation monitor R-29B was discovered inoperable during a routine surveillance check by a plant operator. A Mandatory Limiting Condition for Operation (LCO) was written requiring restoration under work order (SNC344675) to operable status by November 12, 2011 at 01: 18. The work was scheduled improperly and R-29B was not restored to operable status within 7 days. A Condition Report was generated and causal determination (CAR 192866) assigned to identify the cause and take corrective actions to prevent recurrence.

Maintenance subsequently found Channel 7 (Mid Range Noble Gas) of R-29B reading "Out for Maintenance" because of a computer micro processing issue.

The computer was reprogrammed and indication restored to normal. Operations performed their surveillance check satisfactory and restored R-29B to operable status on November 17, 2011 at 15:27.
