APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-353 Limerick Generating Station License No. NPF-84 Unit No. 2 As a result of the inspection conducted on July 31, 1989 - August 4, 1989, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (Enforcement Policy), 53 Federal Register 40019 (October 13,1988), the following violation was identified:

10 CFR 50, Appendix B, Criterion VI, " Document Control," requires that measures be established to control the issuance of procedures, including changes, and that these procedures including changes, be reviewed for adequacy and approved for release. These requirements are implemented by s administrative procedure A-2. " Control of Procedures and Certain Documents." \

Contrary to the above, surveillance test procedure ST-6-043-200-2,

" Reactor Recirculation System Quarterly Valve Test," Revision 2, dated June 13, 1989, contained errors from the revision process, and no review was performed prior to its issuance and use on August 3,1989. In addition, on August 4, 1989, a controlled copy of this procedure did not contain the most recent revision which had been approved two months earlier.

These deficiencies are collectively a Severity Level V Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, considerations will be given to extending this response time.
