. APPENDIX _A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-352 Limerick Unit 1 License No. NPF-39 As a result of the inspection conducted on May 17 - June 30, 1987 and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

10 CFR 50 Appendix B Criterion V requires that " activities affecting quality shall be prescribed by documented procedures and shall be accomplished i accordance with these procedures."

Administrative Procedure A-41.1, Troubleshooting Safety Related and Tech Spec Equipment, requires initiation and approval using a Troubleshooting Control Form when it is necessary to make a change in system configuration while troubleshooting- safety related or technical specification equipment'.

Contary to the above, on June 11, 1987, while rearranging control red blados with the core completely offloaded, two lirit switches for the refueling platform position indication required by Technical Specifications (associated with tha control rod block-interlock with the reactor mode switch in Refuel) were raised without using a Troubleshooting Control Form. 1 This is a severity level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further vlo1ations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time, I
