t APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Co Docket No. 50-353 Limerick Unit 2 Nuclear Power Plant License No. CPPR-107 As a result of the inspection conducted during the period April 25 through May 3, 1988 and in accordance with the General Statement of Policy and Procedure for NRC Enforcement Action,10 CFR Part 2, Appendix C (Enforcement Policy 1988),

the following violation was identified.

10 CFR Part 50, Appendix B Section V states that "Activities affecting quality shall be prescribed by documented instruction, procedures, or drawings of a type appropriate to the circumstances and shall be ac-complished in accordance with these instructions, procedures, or drawings.

Instructions,-procedures, or drawings shall include appropriate quanti-tative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Contrary to the above, on April 25, 1988 licensee personnel were observed resurfacing (lapping) the "0" emergency diesel generator crankshaft main and connecting rod bearing journals without documented instructions to establish proper lapping materials, lapping procedures, and acceptance

  • criteria. As a consequence newly installed bearings were found with substantial residual contaminating abrasive lapping compound left in the 4 bearings.

r This is a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within thirty days of the date of ,

the letter which transmitted this Notice, a written statement or explanation in l reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further i

violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
