APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company ' Docket'No. 50-353 Limerick Unit 2 License No. CPPR-107 l

An NRC inspection co'nducted during the period September 8-18, 1987, identified one violation of NRC requirements. In accordance with the " General Statement '

of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the particular violation is set'forth below.

10 CFR 50.55a requires that construction and inspection be conducted in accordance with the ASME Boiler and Pressure Vessel Code Section III.

Limerick's FSAR, Field Fabrication and Installation Specification.

8031-P-360, and site radiographic procedure RT-ASME III/XI invokes ASME Section III 1974 Edition with addenda through winter 1974.

Section III of the ASME code, paragraph NB 5320, directs that sections of welds that are shown by radiography to have any elongated indication which has a length greater than 1/4 inch, for wall thicknesses up to 3/4 inch inclusive be rejected.

Contrary to the above, an NRC review of pipe weld radiographs during the period of September 8-18, 1987, disclosed that inspectec. and accepted ASME  !

Class 1 weld No. DLA 212-2FW4 contained a rejectable elongated indication 3/8"  !

in length which is greater than the 1/4" allowable for thicknesses up to 3/4" inclusive. This indication was not recorded or dispositioned.

This is a Severity Level V violation (Supplement II) and was identified for j Unit 2, Docket No. 50-353. )

Due to the corrective actions taken by Philadelphia Electric Company and reviewed by the inspector prior to completion of the inspection, Philadelphia' Electric-Company need not respond to this violation. The corrective actions taken are discussed in the enclosed NRC Inspection Report 50-353/87-12. (Enclosure 2)
