APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Limerick I Docket No. 50-352 License No. NPF39 As a result of an inspection conducted on March 29-April 23,1989, and in violation was identified:accordance with NRC Enforcement Policy (10 CFR 2, Append Plant Technical Specification 6.8.1 requires written procedures to be activities. implemented ard maintained to control various plant established, Maintenance Procedure M-041-07 provides measures for the control of work on the Main Steam Safety Relief Valves to ensure the work is performed properly and damage to the valves does not occur during valve disassembly and reassembly.

Contrary to the above, during the disassembly of Main Steam Safety Relief Valve PSV-041-1F013L, damage occurred to the valve as a result of failure to properly adhere to procedural cautions, and the unauthorized use of a hydraulic jack which was not specified by the procedure.

This is a Severity Level IV violation (Supplement 1)

Pursuant to the provisions of 10 CFR 2.201, Philadelphia Electric Company is hereby required to submit to this office within thirty days of the date of the letter which reply, transmitted this Notice, a written statement or explanation in including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
