ENCLOSURE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket No. 50-328 Sequoyah License No. DPR-79 During the Nuclear Regulatory Commission (NRC) inspection conducted on May 4-6, 1987, a violation of NRC requirements was identified. The violation involved installation of undersized snubbers in four pipe supports. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"

Criterion V of 10 CFR 50, Appendix B, as implemented by TVA Nuclear Quality Assurance Manual Part 1, Section 2.5, requires that activities affecting quality be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

Contrary to the above, Basic Engineers pipe hanger drawings for pipe support numbers 2RCH-184, 2AFDH-229, 2CVCH-154, and ECSH-457 were not appropriate. This resulted in installation of undersized snubbers in these pipe supports. That is, the hanger design loads exceeded the rated load capacity of the installed snubbers.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
