ENCLOSURE 1 NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-327 and 50-328 l
Sequoyah Units 1 and 2 License Nos. DPR-77 and DPR-79 During an NRC inspection conducted June 6 through July 10, 1993 violations of NRC requirements were identified.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violations are listed below:

Technical Specification 6.8.1 requires, in part, that written procedures i
be established, implemented and maintained for applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Quality Assurance

Program Requirements, Revision 2, February 1978. This includes procedures required for the safe operation and ref!eling of nuclear j
power plants.

Site Standard Practice SSP-12.1, CONDUCT OF OPERATIONS, Revision 5, Paragraph 3.1.2 discusses the responsibilities of the Operations department positions.
Step 3.1.2.M.8 of SSP-12.1 requires that the fuel handling supervisor (a licensed Senior Reactor Operator) ensure that all work is performed in a safe manner by the appropriate procedure.
Contrary to the above, on June 21, 1993, a fuel handling supervisor did

not perform the evolutions of FHI-3A, RECOVERY OF LEANING FUEL ASSEMBLY H64, in accordance with the requirements of SSP-12.1 in that weight limits imposed for the evolution were exceeded on two occasions prior to-i the evolution being halted by plant management. The violation occurred j
during evolutions involving the recovery of a tipped fuel assembly in
_j the Unit I reactor vessel.

10 CFR Part 50, Appendix B, Criterion XVI. Cnrractive Action, requires, i
in part, that measures shall be attablished to assure that conditions adverse to quality such as nonconformances are promptly identified and corrected.
In the case of fignificant conditions adverse to quality, I
the measures shall assure tie cause of the condition is determhed and I
i corrective action taken to preclude repetition.

In November 1991, JJring the Unit 1 Cycle 5 refueling outage, licensee reactor vossel inspections identified foreign material an

the lower core plite during fuel reload.
Corrective actions were taken to remove t'se foreign material and to preclude future j
problems. On Jure 19, 1993, during the Unit 1 Cycle 6 refueling outage, foreign aaterial on the lower core plate resulted in tipping of a fu el assembly during fuel reload.

Contrary to the requirements of 10 CFR 50, Appendix B, Criterion XVI, corrective actions taken for the Unit 1 Cycle 5 foreign material event did not preclude repetition of a condition adverse to quality.

Licensee Event Report 327/92-21 described an event where, on November 20, 1992 the licensee failed to perform verification of the position of 10 manual containment isolation valves as required by Technical Specifications.
The licensee stated in the LER response that a similar problem was identified on January 27, 1992 in LER 327/92-03, where they failed to verify valves in the required position in accordance with Technical Specifications.
Corrective actions for_LER 327/92-03 included review of the l
procedure that was determined to be inadequate in LER 327/92-21.
Contrary to the requirements of 10 CFR 50, Appendix B, Criterion XVI, corrective actions for the LER 327/92-03 were inadequate to identify additional valve lineup problems and institute prompt corrective actions.
