NOTICE OF VIOLATION 1

k Tennessee Valley Authority Docket Nos. 50-327, 50-328 l Sequoyah License Nos. OPR-77, DPR-79 During the Nuclear Regulatory Commission 1NRC) inspection conducted from May 6, 1989, through June 5,1989, violations of NRC requirements are identified. In accordance with the " General Statement of Policy and Procedure for NRC ,

Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violations are '

listed below:

A. TechMcal Specification 5.8.1 requires that procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, be established, imple-mented and maintained. This includes maintenance procedures. The requirements of TS 6.8.1 are implemented in part by the following procedure:

IMI-134, Configuration Control of Instrument Maintenance Activities, states that the technician performing a work activity shall list and initial any configuration changes in order on the IMI-134 data sheet.

These changes include jumpers, wire lifts, inhibits, temporary instrument settings, unbolting flanges, disconnecting tubing and pipe fittings, temporary connections, etc. The technician is to list these items in sufficient detail to uniquely identify each item.

Contrary to the above, the configuration of RM 90-404 with respect to the detector cable was not correctly controlled as required by IMI-134 in that configuration changes had been made without initiating the changes on the IMI-134 data sheet.

This is a Severity Level IV violation (Supplement I).

8. Technical Specification 6.8.1 requires that procedures recommended in l Appendix A of Regulatory Guide 1.33, Revision 2, be established, l implemented and maintained. This includes maintenance, operating, i surveillance, and fuel handling procedures.

l Contrary to the above, the licensee failed to establish and implement j adequate procedures for power operation to niaintain Thermal Power within 1 license limits during steady state operation. i This is a Severity Level V violation (Supplement I).

DPR-77, DPR-79 Reactor Regulation, and a copy to the- NRC Resident Inspector, Sequoyah, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include [for each violation): (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results ' achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.

Where good cause is shown, consideration will be given to exteMing tha response time. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper l should not be taken.
