b ENCLOSURE 1, NOTICE OF VIOLATION
. Tennessee Valley Authority Docket Nos. 50-327, 50-328 Sequoyah License Nos. DPR-77 DPR-79 1
During the Nuclear Regulatory Commission (NRC) inspection conducted September. 6.
1990, through October 5,
1990, - a violation of NRC requirements was identified.
In accordance with the' " General Statement of. Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, AppendixC:(1990),theviolationislistedbelow:
Technical Specification 6.8.1 requires that procedures recommended in Appendix-A of Regulatory Guide 1.33,_ Revision 2, be established, implemented and maintained.-
This includes maintenance, operatira,
')
-surveillance,- administrative, and fuel handling procedures.
Administrative Instruction AI-18.78, Post-tri) Review, requires a root cause determination to be complete.and the Post-Trip Review to be approved by the Plant Operations Review Comittee prior to restart.
Contrary to the above, the _ requirements of AI-18.78 were not implemented in that a root cause of a; reactor trip was not identified in the Post-trip Review Report and approved by the Plant Operations Review Committee prior to restart of Unit 1 on September 19, 1990.
This is a. Severity Level IV. violation (Supplement I)
Pursuant' to the-provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit' a written statement' or explanation to the Nuc1 car'~ Regulatory Commission, ATTN:
Document Control Desk, Washington, DC.20655, with a copy to the Regional. Administrator.. Region II, and a cosy-to the NRC Resident Inspector, Sequoyah, within 30 days of the date of t1e letter transmitting this Notice. This reply should be clearly marked as a
" Reply' to-a. Notice of Violation"'and should include [for each violation]:
(1) admission ci-denial-of the violation, (2) the reason for the violation "y.
if. admitted, -(3) the corrective steps which have been taken and the resultsiachieved ' (4) the corrective steps which will be taken to avoid-3 I
further violations, -and (5) the date. when full compliance will be achieved.
Where good cause~ is shown, consideration will be given to.
- extending the response time.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why'the license should not be modified, suspended, or revoked or why such other action as may be proper-should not be taken.
