50-328 SEQUOYAH NUCLEAR PLANT - NRC ANNUAL OPERATING REPORT, JANUARY 1, 1986, TO DECEMBER 31, 1986 - OPERATING LICENSES DPR-77 AND DPR-79 As required by the Sequoyah Nuclear Plant Technical Specifications, TVA is submitting the annual operating report for Sequoyah units 1 and 2 for the period January 1 through December 31, 1986.

As stated in-the cover letter to NRC with the 1985 annual operating report, Sequoyah, in an effort to improve regulatory performance, is revising the methodology for reporting information in the annual operating report.- This

. effort has resulted in the following:

A.

Facility Changes - A report of facility changes pursuant to 10 CPR 50.59(b), including a summary of the safety evaluation of each.

This year's report contains the facility changes reported by TVA for the Final Safety Analysis Report (PSAR) update. In previous years, facility changes were reported in the annual operating report when work was field complete. Since the FSAR update reports totally closed Engineering Change Notices (BCNs),-those reported were cross-checked with the last three years' annual operating reports.

Those ECNs previously reported were identified and are not included in this report..A summary of the safety evaluation of each facility change is included.

B.

Temporary Alterations To The Facility (temporary alteration control forms - TACFs).

Temporary alterations to the facilities that were open at the end of the year and have been in existence, or are expected to be in existence, greater than a year are reported in this report. A summary of the safety evaluation for each temporary alteration reported is included.

Reporting TACFs based on the above criteria conforms with the FSAR update reporting requirements.

Procedure Changes - Report of procedure changes pursuant to-10 CFR 50.59(b), including a summary of the safety evaluation of each.

TVA is in the process of identifying the procedures that are described in the FSAR. As a minimum, all revisions to technical specification surveillance instructions, abnormal operating instructions (AoIs), and emergency instructions that (1) made a significant change to the scope, technique, or sequential order of steps affecting results or nuclear safety; (2) waived or violated i

technical specifications or licensing requirements; (3) affected a QC hold point; (4) changed or revised the review / approval authority or responsibility; (5) implemented a temporary alteration.to operate critical Structures, Systems and_ Components (CSSC) equipment; or (6) changed the acceptance criteria are reported in this report.

Revisions _to correct typographical errors and general administrative changes are not reported. A safety summary of all reported revisions is included. Additional procedures may be included in future reports if they are identified as described in the PSAR.
