1.0 PURPOSE .

t This instruction defines the methodology and requirements to be utilized for performing administrative reviews of completed DAP Type C checklist packages, (Ref. DAP-4, Design Review Evaluation Checklist, Attachment C). The purpose is to ensure that the approved checklist packages have been completed in accordance with other DAP procedural requirements and provide the mechanism for documenting any necessary corrective action. It provides guidance for entering information into a data base which will be i

utilized to cross-reference pertinent data (i.e., HDA number (s), DIR numbers, criterion, etc.) in order to comply l with a commitment made to the NRC.

2.0 SCOPE i

,i This instruction addresses the review of Type C checklist packages in the Civil / structural self-initiated review area that were completed and approved prior to 10/1/86. This instruction addresses the format and documentation of information contained in the checklist package and does not address technical review.

The activities described in the following sections shall be performed

to review the compliance of completed Type C checklist packages with DAP requirements for all checklists that were completed and approved prior to 10/1/86. l l

4.2 Performance of Administrative checklist Review Attachment A or a checklist containing equivalent information shall be used to document the admini-strative review discussed in Section 4.1. Step-by-step instructions for completion of this i

checklist are included in the attachment.

All items on the A-1 Form should be designated as

l ATTACHMENT A INSTRUCTIONS FOR REVIEW OF COMPLETED CHECKLIST USING FORM A-1 GENERAL INSTRUCTIONS

1. Review the completed Type C checklist package using the following Detailed Instructions and the Adminis-trative Review form (Form A-1) . Each UNSAT or N/A listed on the form must be explained.
2. If UNSAT items are found, a revision to the Type C checklist package will be prepared to correct these items. This will be done in accordance with procedure DAP-4, Section 5 and this instruction. The revised and added pages with a Revision Approval Cover Sheet will form the revision package. Do not alter the record copy of the previous revision of the checklist package in any way as it will be retained for perma-nont record.
3. The completed revision package along with the completed Administrative Review form (Form A-1) shall be given to the Discipline Coordinator for review and approval.

O INSTRUCTIONS FOR FORM A-1 " RECORD VERIFICATION CHECKLIST" Item 1. Use of Approved Checklist Form: (DAP-4, Sec-r tion 5.1)

Approved Type C checklist forms must be used in the reviews. Check if the form number (includes date and revision) corresponds to the list of approved checklist forms. If an unapproved checklist form was used, obtain approval of the form following DAP-4, Section 4.4 or initiate other action to resolve the UNSAT as instructed by the Discipline coordinator.

Item 2. Checklist Control No.: (DAP-4, Section 5.3)

All DAP-generated pages in the Type C checklist package must be identified with the checklist control number (DAP-CLS-C/S-YYY). The first page of source or reference documents that are included in the package must also be identified with the same control number.

Item 3. Checklist / Criteria The Type C checklist package must contain a correlation of criteria numbers (from the C/S criteria list) to the checklist attributes.

(Note: This information, if missing, will be collected in accordance with Attachment B and included in a revision to the Type C checklist package.)

Item 4. Checklist /HDA correlation (DAPs-5 and -6, Section 4.3)

The Type C checklist package must contain a correlation of HDA numbers to the checklist attributes. ,

Item 5. Reviewers, Checkers and Approver Qualification

For the production of Type C checklist packages, O adequate qualification means completed reading assignments for procedures DAP-2, -4, -5, and -6.

Check dated signatures on the cover sheet against list of qualified personnel with dates.

First, the correlation will be shown by the addition of D-XXXX numbers in the Attach-ment B data base information. In this process, each UNSAT Attribute should be compared to the D-XXX DIR wording to - make sure the correct D number is used. The second step involves crossing out (do not obliterate) the interim DIR numbers contained on the UNSAT Checklist Summary Sheet within the review package and writing the D-XXX DIR number immediately adjacent to the interim number. (See Instruction 10 on revi-

, sions.) During this second step, any updates on the UNSAT Checklist Summary can be made to

reflect mergers or other changes in DIRs if these changes are not reflected on the Summary Sheet.

Notes: (1) Where signatures need to be obtained, they will be obtained for the revision on the " Calculation Review Cover Sheet" and a note will be added to clearly state that " signatures on this page constitute approval of the entire checklist package." (2) Where DAP review comments have been made directly on the source calculations that are part of the Type C checklist package a note should be added to the front page of the calcula-tion. The note should indicate "This copy contains DAP review comments." A revision symbol (See Iten 10) shall be placed next to the note on the front page.

Item 8. Design Checklists Are complete

Item 10. Table of Contents Identifies Items in Package .

[ All items contained in' the Type C checklist package (including all revisions) must be identifiable from the cover sheet or attached pages in sufficient detail to determine that they' are part of the package. Any item which consists of an excerpt from a document shall be identified using the identification sheet shown in Attach-ment A to DAP-14.

Note: If this is found to be UNSAT, use the attached form A-2'or equivalent in preparing the required revision.

Item 11. Preparation of Revisions 1

A revision package is to be prepared to correct any UNSAT items noted in this review. This j package shall consist of the following:

O These forms have bean prepared for the latest approved revision of each Type C checklist form. For the review of Type C check-list packages, the reviewer should obtain one form for each checklist form number that is contained in that package.

If reviewers encounter checklist forms for an earlier revision level, they should first mark up a copy of the B-2 form for the latest revision to reflect differences in attribute, criteria, and/or constant HDA structure. This markup will then be input to the data base, in the same manner as Step A above and pre-printed forms fer that revision will be produced for use in the review of the Type C checklist package.

Final information for each Type C checklist package review will be input into the data base, printed out, and proofed by the _

reviewer.

C. Final Outnut/ Checks After all data is input into the computer, a printout similar to the format of Form B-2 will be prepared. This will be spot-checked by an independent reviewer (checker). Any corrections will be noted and made. The final output will then be printed for inclusion in the revision to the Type C checklist package.
