, QI-005 Revision 3 O ITEM CHANGE TO QUALITY INSTRUCTION

1. Page 4, Attribute 5.3 Delete
2. Page 4, New Attribute 5.3 to read:

"Upon completion of the matrix the inspector shall complete the Data Base Report, '

Attachment 6.2."

3. Page 4,' Attribute 5.4 to read:

"For purposes of tabulating inspection results, the information from the Data Base Reports will be accumulated and documented on the Decision Point -

Suasary Sheet, Attachment 6.3.

4. Add Attribute 5.4.1 "The Decision Point Summary Sheet shall be completed for each inspector reviewed in accordance with Attachment 6.4. Decision Point Summary Sheet instructions.
5. Attribute 5.5 (previously 5.3) to read: ,

"The ERC Discipline Level III will tabulate O the reinspection results for each inspector.

As a minimum, the results tabulation will identify":

a. Total number of attributes / decision points reinspected,
b. Total number of disagreements per objective attributes.
c. Total number of disagreements per subjective attributes.
d. Results of review per 4.2 above.
6. Attribute 5.6 (previously 5.4) to read:

" Final tabulations will be provided to the I.d.1 Issue Coordinator."

4. Enter the number of items inspected for that attribute. -
5. To calculate total no. of decisions, multiply the decision item x no. of items.
6. From the comparison between the original inspection report and the Phase III reinspection report, enter the no. of disagreements for each attribute as applicable.
