i This procedure provides the methods and the accept / reject criteria for performing documentation review for epoxy grout. 2.0 APPLICABILITY This procedure applies to the samples selected from the Epoxy Grout population which includes grout placements made in Unit 1, Unit 2, and Common areas that are safety related, construction complete, and QC accepted.

3.0 REFERENCES

]i 4.0 GENERAL

[ (-~) Documentation reviews are to be performed and documented in accordance with established CPRT procedures and instructions. This procedure establishes instructions, attributes, and the accept / reject criteria to perform the Documentation Review of Epoxy Grout Reference 3.1 addresses the methods to perform, document,.and process the results. l 5.0 INSTRUCTIONS Using the information below, perform the document review of the selected sample items and document the results on the applicable checklist (Attachment 6.1). The Epoxy Grout Inspection Records to be reviewed are located in the TUGC0 Records Vault or the Brown & Root Interim Records Vault. If the inspection records are not located the inspector shall contact the QA/QC Discipline Engineer. The Inspector shall review the Grout Placement package to determine which attributes are applicable. Where no inspection verification exists for applicable attributes, DRs will be initiated.

1. Preplacement Verify that the following preplacement requirements were satisfactorily noted as accepted on the checklist.

O' a. Verify the gaps between equipment base and concrete, or baseplate and concrete meets requirements. l 2

b. Verify grout placement holes are properly placed.
c. Surfaces to receive grout are free of foreign matter and surfaces are dry.
2. Placement-Verify that the following placement requirements were satisfactorily noted as accepted on the checklist.

a. Surface temperature of'the area to be grouted with Ce11 cote epoxy grout is a minimum 50' F. For Escoweld epoxy grout the ambient temperature is 60*.F or above.
b. Mixing operation of epoxy grout.
c. Placing is continuous with the pot life of the grout ,

not exceeded.

3. Post Placement Verify that the following post placement requirements were satisfactorily noted as inspected on the checklist for cure time and on the test results data sheet for compressive strength respectively.

a. The epoxy grouted areas have not been placed into service prior to the required cure time,
b. The lot of epoxy grout was tested to the requirements of AS,TM C-579. The epoxy grout shall have "during use" l compliance test results meeting a minimum compressive strength requirement of 12,000 PSI.
4. Inspector and Test Personnel Certification Verify that each inspector who signed any item on the test results or the Inspection Report was certified as follows: ,
a. The inspector was certified to the procedure at the time of inspection, i.e. the date that the inspector initialed / signed must be on or af ter his certification date and prior to his certification expiration date.

If this attribute is rejected determine if 

                                       ,                                                          the person that signed as QC inspector was ever certified to the procedure used. If the inspector was ever certified to the procedure, record the time period of certification on the DR, otherwise state on the DR that the person was never certified.

NOTES: Inspector's name shall be recorded on the DR when a j deviation occurss_ . If any initials and/or signatures are illegible, an attempt shall be made to determine the inspector's name , by checking the signature cards located in the Permanent Plant Records Vault. If identification cannot be made the inspector shall reject this attribute. , 1 Basis for verification of certification is the TUGC0 Inspection Certification Form.
