Box 220 St. Francisville, Louisiana 70775 SUBJECT: NRC INSPECTION REPORT NO. 50-458/98-10 Dear Mr. Edington: This refers to the inspection conducted on February 22 to 26,1999 at the River Bend Station facility. The purpose of the inspection was to review radiation work permit program implementation from October 1997 to February 1999 and the problem identification and resolution program as it applied to the radiation work permit program deficiencies. A followup discussion was conducted on March 25,1999, to address a change to the findings presented during the inspection exit meeting. We concluded that the radiation work permit program was implemented poorly until July 1998 because the program imp menting procedures provided inadequate guidance. As a result, radiation work permits contained little radiological information, work areas were not specifically addressed, revisions were not communicated to workers, locked high radiation area work permits did not specify work area dose rates or maximum allowable stay times, and protective clothing requirements were implemented inconsistently. While we did not identify any actual safety consequences stemming from these implementation problems, the problems noted above could result in inadequate or incomplete instructions to radiation workers, erroneous - dose assignment data, or an increased potential for personnel exposure or contamination. Additionally, we concluded that you did not identify these multiple, long-standing radiation work [[/- permit program weaknesses until questions were raised by NRC. However, once initiated, your , assessment was generally good; although, additional radiation work permit problems were identified by your staff and NRC. Based on the results of this inspection, the NRC has determined that a violation of NRC requirements occurred. This violation is being treated as a Non-Cited Violation (NCV), consistent with Appendix C of the Enforcement Policy. The NCV is described in the subject inspection report. If you contest the violation or severity level of the NCV, you should provide a response within 30 days of the date of this inspection report, with the basis for your denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington DC 20555- 0001, with a copy to the Regional Administrator, Region IV, and the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001. In addition to the NCV, one unresolved item was identified concerning instruction to radiation workers in precautions and procedures to minimize exposure to radiation. This review will be conducted during the refueling outage that is scheduled to commence in April 1999. Other aspects of your continuing corrective actions stemming from your assessment of the radiation work permit program will be conducted in conjunction with this review. in accordance with 10 CFR 2.790 of the NRC's ' Rules of Practice," a copy of this letter and its enclosure will be placed in the NRC Public Document Room (PDR).
