APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company _

Docket: 50-382 Waterford Steam Electric Station, Unit 3 Operatirg License: NPF-38 During an NRC inspection conducted on May 31 through June 3, 1988, a violation of NRC requirements was identified. The violation involved failure to follow a Technical Specifications (TS) requirement for procedure change. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Failure to Follow TS Requirement for Procedure Change Paragraph 6.8.2 of the TS specifies that each procedure required by TS 6.8.1, and changes thereto, shall be reviewed as specified in TS 6.5.1.6 and shall be approved by the Plant Manager-Nuclear prior to implementation and reviewed periodically as set forth in administrative procedures.

l Contrary to the above, during performance of the Isothermal Temperature Coefficient Measurement Procedure, NE-2-060, with control rod reference group "B" inserted, the pro:edure required by TS 6.8.1, was changed without approval from the Plant Manager-Nuclear. Specifically, instruction 6.2 specifies that the RCS temperature be decreased 5 to 10*

at a rate less than 50 F/ hour, but cooldowns were successively conducted at 3.9", 2.8 , and 2.0'F.

letter transmitting this Notice, a written statement or explanation in reply, l including for each violation: (1) the reason for the violation if admitted.

! (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
