APPENDIX A NOTICE OF VIOLATION Entergy Operations, Incorporated Docket No..

50-382 Waterford Steam Electric Station, Unit 3 License No.: NPF-38 During an NRC inspection conducted on March 24 through April 1, 1993, a violation of NRC requirements was identified.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:

Technical Specification 6.8.1.f requires that written procedures be established, implemented, and maintained covering implementation of the Fire Protection Program..7 of Administrative Procedure UNT-005-013, Revision-12,

" Fire Protection Program," requires, in part, that, with one or more fire rated assemblies and/or sealing devices inoperable, an hourly fire watch patrol be established.

Section 4.5 of Fire Protection Procedure FP-001-014, Revision 7, " Duties of a Fire Watch," requires, in part, that each fire watch patrol observe areas for the purpose of detecting, reporting, and acting on fire hazards.

Contrary to the above:

- On March 24, 1993, the fire watch patrol in the Valve Operating Enclosure Bay A room was inadequate in that the personnel performing the patrol failed to observe the area for fire hazards.

- Fire watch patrols conducted on March 7,13, and 14,1993, in Emergency Diesel Generator Room B; conducted on March 13, 1993, in Emergency Diesel Generator Room A; and conducted on March 7 and 13, 1993, in the Valve Operating Enclosure Bay B room were inadequate in that the time duration that patrol personnel were in these rooms was insufficient to observe the area for fire hazards.

This is a Severity Level IV violation (Supplement I)(382/9310-01)

Pursuant to the provisions of 10 CFR Section 2.201, Entergy Operations, Incorporated, is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident inspector, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be 9305110121 930504 PDR ADOCK 05000382 G

If an adequate reply is not received within the time specified in l

this Notice, an order or demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.
