ENCLOSURE 1 NOTICE OF VIOLATION Entergy Operations, Inc.

NPF 38 During an NRC inspection conducted on September 22 26 and October 6-10,1997, a violation of NRC requirements was identified, in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the viola 1 Pan is listed below:

A:

10 CFR Part 50, Appendix B, Criterion V, " Instructions, Procedures, end Drawings,"

states that activities af'ecting quality shall be prescribed by documented 1,m instructions, procedures, or drawings, of a type appropriate to the circumstances 9;

and shall be accomplished in accordance with these instructions, procedures, and j

drawings.

Site Procedure W4.503, " Changes to the Technical Specifications, Technical Requirements Manual, or Core Operating Limits Report," Revision 4.1, required that desired changes to the technical requirements manual be evaluated under the 10 CFR 50.59 process in accordance with Procedure W2.302. "10 CFR 50.59 Safety and Environmental Screenings," and reviewed by the mt operating review committee.

Procedure W2.501, " Corrective Action," Revision 6, states that an individual who identifies a condition adverse to quality shallinitiate a condition report.

Contrary to the above, on April 8,1996, desired changes ware identified for entries in Table 3.8-1 of the technical requirem Tts manual and as of October 10,1997, had not been evaluated und - the 10 CFR 50.59 process.

Contrary to the above, the licensee f ailed to initiate a condition report following the discovery in 1997 of a condition adverse to quality involving Calculation EC-191-027, Revision 0, dated December 30,1991, which analyzed the error associated with the containment spray riser '.evel gauges.

The calculation, which did not account for density changes in the water column being measured, affected the calculation of peak containment pressure for a loss-of-coolant accident.

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV,611 Ryan Plaza Drive, Suito 400, Arlington, Texas 76011, and a copy to *he NRC Restnnt inspector at the f acility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
