,, May 19, 1977 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--FNO-77-87 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evalultion and is basically all that is known by IE staff on this date.

Subject:

FAILED FUEL On May 15, 1977, during refueling operations, the licensee identified two (2) failed fuel bundles with parts of the fuel rods missing. One Fuel Bundle. identified as SN 2-43, which was located near the center of' the core, had a six-inch piece of cladding'~ ~from an outer fuel rod missing.

The piece was later found in the spent fuel pool well. The other bundle, identified as SN 2-39 was located near the core periphery and was found to have two pieces of fuel rod cladding missing. The pieces were found resting on top of an adjacent fuel bundle. The licensee is developing a procedure for recovery of the pieces on top of the core.

mechanism is being constructed to capture any additional pieces of failed - -
