ULNRC-7 98 INTERIM 10 CFR 50.55(e) REPORT U-76 BETTIS ACTUATOR KEYING PROBLEM WITH FISHER VALVES CALLAWAY PLANT On March 15, 1984 Union Electric Company advised the NRC Region III of a potential significant deficiency under the criteria of 10 CFR 50.55 (e). The potential deficiency involves the dis-association of the key in a Bettis valve actuator from the keyway on the valve stem of a Fisher valve, thus rendering the valve inoperable. The valve serves as a temperature control bypass valve on the "B" component cooling water (CCW) heat exchanger. The valve's function is to regulate the cooling water supply temperature to the component cooling water system-serviced equipment. An identical valve is installed in the "A" CCW heat exchanger. In the event of a safety injection signal, these valves close directing all flow through the heat exchangers. Similar valves are provided elsewhere in the CCW system, containment purge system, and chemical and volume control system.

The cause of the key disassociation between the valve and actuator has not yet been determined. Inspection of all affected valves has been performed by the vendor,and the results of this inspection are currently incomplete pending vendor analysis. This deficiency will be resolved and a final report submitted prior to fuel load.
