LOOSE-PART DETECTION SYSTEM CIIANNEL )

INOPERABLE FOR GREATER TIIAN 30 DAYS I

This report is submitted in accordance with Final Safety Analysis Report 16.3.33.5 Action a, which states:

"With one or more Loose-Pan Detection System channels inoperable for more than 30 days, prepare and submit a Special Report to the Commission pursuant to Technical Specification 6.9.2 -

within the next 10 days outlining the cause of the malfunction and the plans for restoring the channel (s) to OPERABLE status."

At 2000 CDT, on 5/2/98, Channel 3 of the Loose-Part Detection System was declared to be inoperable.

The plant was in Mode 2 at 0% power.

The Channel 3 accelerometer is located in containment mounted on the reactor vessel head assembly. The problem is loss of signal due to failure of a component inaccessible at power (charge converter sensor).

Additional troubleshooting will be performed during the next outage of suflicient duration. Channel 4, also on the reactor head assembly, remains operable as well as the remaining ten channels in the Loose-Part Detection System.

Until further troubleshooting is completed, actions to be taken to prevent recurrence can not be determined.
