A potential problem related to terminal blocks which we may have supplied as spare parts was recently determined in our plant.

Connectron manufactures NU2 terminal blocks out of nylon and polysulfore materials. To our knowledge only the polysulfore material has been IEEE qualified for application in our terminal boxes furnished with our hydraulic actuators. These actuators are installed on safety related valves.

We are quite certain that the original equipment furnished to our customer had the correct terminal block material and therefore this problem only pertains to spare parts that we may have furnished.

All customers have been notified of this potential problem and a typical letter is attached for your information. The following plant sites have equipment that was shipped with the correct terminal material and may have spare terminal blocks furnished with wrong material:

The subject equipment supplied on the referenced order has been supplied with Connectron tenninal blocks Model #NU2. Connectron manufactures the NU2 terminal blocks out of nylon and polysulfone materials. However, only the polysulfone material has been environmentally IEEE qualified for the subject application.

The originally supplied equipment contained the polysulfone NU2 terminal blocks but any subsequently A/DV supplied spare (replacement) terminal blocks / have been made of nylon or polysulfone.

A/DV recommends that a review of the subject equipment, especially spare parts, be made to verify that all terminal blocks are made of polysulfone material. A/DV's part number for the Connectron Model #NU2 terminal blocks is W-30164, W-30211 or W-32676. Use of the nylon terminal blocks would nullify the IEEE qualification of Class lE electrical control and instrumentation circuits of the subject equipment.
