Commonwealth Electric Company

.. Northern States Power Company Monticello Nuclear Plant Monticello, MN 55362 and four (4) rolls of incorrect insulating tape to:

, Ulilhnl Electric Company Callaway Station

,. Portland, MO 65067 _

The naterial was shipped en June 21 & 28, 1984 respectively and the mistake discovered by us on DecemM.r L7, 1984. The Kerite Company promptly notified both utilities and det'erminei'the disposition of the material and that there were no $mminent safety concerns.- Immediate arrangements were made to replace the incorrect tapes. ,

The specific dispasition of thC tapes at Monticello and Calloway was as follows: ,

Monticello: The material Aad been used to connect, Kerite cables to four residual heat removal pumps'and two containment spray pumps. There was to apparant safety problem because the plant was down"for refueling and had not

. gone back on line.

, l During the week of December 30, 1984, Kerite engineering personnel supervised the remaking of these connections using the properly qualified tapes.

Calloway: The tapes ~had not tc,en used and were returned to Kerite during

The nuclear qualified tapes, and others, are purchased from an outside vendor.

We had ordered a shipment of one kind and size of non-nuclear qualified tapes and found that mixed in the shipment were different. size tapes. All tapes were

. received unlabeled, contrary to our own specifications and practices. In the sorting of the tapes, an identification error was made and the rolls of non qualified tapes were incorrectly labeled " Insulating Tape, Type 1" and put into stock.

A Kerite engineering technician had occasion to draw Type I tape from this stock and discovered that the material did not have the properties characteristic of this type of tape. He reported this to Q.A. and subsequent tests confirmed that the tape was non qualified Bi-Seal III and not the qualified Type I.

WHAT STEPS HAVE BEEN TAKEN TO PREVENT A RECURRENCE?

A review of the events leading up to this problem disclosed that Kerite's instructions governing the handling of incoming materials are fully adequate and,-if followed, would have prevented this error. All persons involved have been cautioned and reminded of the importance of strict adherence ' to Q.A.

procedures and Company rules.
