J. M. Farley Nuclear Plant NRC Inspection of October 11 - November 10, 1983 RE: Report Numbers 50-348/83-29 50-364/83-27

Dear Mr. Lewis:

This letter refers to the violations cited in the subject inspection reports which state:

"As a result of the inspection conducted on October 11 - November 10, 1983, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified.

A. Technical Specification 3.4.10.1, applicable at all times, states, 'The Reactor Coolant System (except the pressurizer) temperature and pressure shall be limited f n accordance with the limit lines shown on Figures 3.4-2 and 3.4-3 during heatup, cooldown, criticality, and inservice leak and hydrostatic testing with ...b. A maximum cooldown rate of 100*F in any one hour period. '

Contrary to the above, the licensee exceeded the Technical Specification in that on November 1, 1983, from 5:00 to 6:00 a.m., the coold&dn rate of the Reactor Coolant System was 113*F as indicated by the computer log."

- operators were' not using all available means of temperature indication to _ verify that the cooldown limits .were not being exceeded. Further, they were plotting 'the cooldown once per hour, as allowed by Technical Specifications, instead of once per half hour as specified by plant procedures.

Corrective Action Teken to Avoid Further Violations Appropriate personnel were counseled to adhere to plant procedures, to monitor cooldown limits closely and, when approaching the Technical Specification limit of 100* in one hour, to use other available means of temperature indication to verify cooldown limits are not exceeded.

Corrective Steps Taken to Avoid Further Violations See above.

"B. Technical Specification 6.8.1.c requires that written procedures shall be established, implemented and maintained covering surveillance and test activities of safety related equipment.

Contrary to the above, FNP-0-STP-26.0, " Control Room Train A Ventilation Operability Test," performed on November 7,1983,

-was inadequate in that the procedure did not verify that the control room pressurization unit heater and the humidity detector, which controls the heaters, were operable as required by the acceptance criteria of Technical Specification 4.7.7.6."

The above violation occurred as described in the subject report.

Reason for Violation This violation was' caused by procedural inadequacy in that FNP-0-STP-26.0 (Control Room Train A(B) Ventilation Operability Test) did not provide a means to positively ensure that the pressurization unit heaters were in operation during the test.
