EPIP-0SC-2-1 FORT CALHOUN STATION UNIT NO. 1 Emergency Plan Implementing Procedure EPIP-OSC-2 Emergency Plan Activation Method for initiating the Emergency Plan from the Operations Control Room after the Emergency Classification has been completed.

I. PURPOSE This procedure provides the instruction to be followed by the Shift Supervisor or his designee when an emergency occurs that requires initiation of the emergency plan.

II. PREREQUISITE A. The emergency has been placed into one of four (4) classificaticas.

! B. An initial action has been taken to ensure the plant is in a safe, possibly shutdown, condition.

III. PRECAUTIONS A. All significant events and action shall be logged in the Operation Log book.

B. The person relieving the Shift Supervisor as Emergency Duty Officer shall be fully briefed prior to assuming the respon::ibility.

IV. PROCEDURE

1. Notify plant personnel.

Notification of Unusual Events: Announce the classification condition twice over the Gaitronic system. Consider evacuation of compartments within the containment and auxiliary buildings.

Alert, Site Area, General: Sound the Nuclear Emergency Alarm followed by an announcement on the Gaitronic with special evacuation instruction. Repeat instruction.

2. Notify the Manager - Fort Calhoun Station or his alternate. The Manager - Fort Calhoun Station or his alternate will then notify 0.P.P.D. management of emergency class and plant status.
3. Assign Emergency Call List notification to management for contact of personnel during off normal working hours.

a. The Recovery Manager, Emergency Coordinator, Plant Operations Manager, and the Emergency Duty Officer agree that the Recovery Organization should be activated for possible emergency escala-tion.
b. The event is elevated to the next higher emergency classifica-tion.

Site Area and General: Those individuals reporting for the Alert emergency will also report for a Site Area and/or General Emergency. The remaining members of the Recovery Organization will be notified to report to their assigned emergency response center as soon as possible.

4. Notify State / local support groups.
a. Contact Nebraska State Civil Defense and Iowa Disaster Services through the (NSP) Nebraska State Patrol - telephone number long distance 477-3951. For Notification of Unusual Event, filow with direct call to Iowa Office of Disaster Services - telephone 515-281-3231/515-281-3561 .

b. The caller will identify himself and ask for the name of the NSP dispatcher. For verification purposes the caller will respond to the NSP dispatcher with the badge number of the dispatcher using the Personnel Listing, Letter dated May 13, 1981. Controlled copies of this confidential listing are maintained in the Fort Calhoun Station Control Room Key Locker.
c. If further authentication is deemed necessary the caller may also provide the initials of the dispatcher provided on the Personnel Listing.
d. If the verification method has not been successful, another NSP dispatcher should be contacted to accomplish verification with the caller.
e. The alternate method of notification is the National Warning System (NAWAS). Verification is not required for this method.

5. Notify the Security Force to confirm Security Emergency Procedures are in effect.
6. Notify the NHC by direct red telephone network within one hour.

(OPPD - Standing Order R-11).

NOTE: Steps 6 thru 11 apply to emergency classes greater than Unusual Events.

6. Provide initial technical information to Nebraska State Civil Defense and Iowa Disaster Services by telephone. (Emergency Notification Form, p.X.I-4 and Nuclear Accident Report Form, p.X.I-6).
7. Complete accountability of Operations personnel. Initiate personnel search and rescue as needed.
8. Direct in plant radiological survey.
9. Direct off-site surveys, as additional emergency monitors arrive.
10. Prepare emergency upd;te information (Message, p.X.I-10 and Environmental Assessment Form, p.X.I-14) . Transmit information to State or County Emergency Operation Facilities if OPPD/ EOF has not yet been staffed.
11. Transfer Emergency Duty Officer (EDO) responsibility with

1 EPIP-OSC-5-1 Fort Calhoun Station Unit No. 1 Emergency Plan Implementing Procedure EPIP-OSC-5 SITE AREA EMERGENCY ACTIONS I. PURPOSE To outline the steps to te taken by members of the Operations Staff when a Site Area Emergency is declared-II. PREREQUISITES A. A Site Area Emergency has been declared on the basis of emergency classification described in EPIP-CSC-1,Section IV.C.

B. The Radiological Emergency Response Plan has been activated as per EPIP-0SC-2.

C. Emergency Procedures EP-1 through EP-37B are available.

III. PRECAUTIONS A. All significant events and actions shall be logged in the operation log book.

B. The person relieving the Shift Supervisor as EDO shall be fully briefed prior to assuming the responsibility, in accordance with ,

EPIP-E0F-13.

IV. PROCEDURE i

1. Shif t Supervisor and/or EDO: Activate the on-site emergency organi-i zation, using EPIP-OSC-2 and EPIP-RR-1.
2. Notify Washington, Harrison, and Pottawattamie County officials in accordance with EPIP-E0F-17 within fifteen (15) minutes of declaring a Site Area Emergency.
3. The Shift Operations Staff shall:

l (1) Monitor the plant conditions and inform the EOF and TSC of the

l l (a) Status of RC system (b) Engineering safeguard system (c) S/G level, temperature, pressure, and FW status.

(d) Shutdown cooling system temperature, and flow.

until the dose assessment coordinator function is filled; J (a) Radioactive release, if any, from the plant and appropriate radiation monitor reading.

(b) Status of other radiation monitors.

(c) Meteorological information such as X/Q, wind speed, wind direction, and air temperature.

Initial dose assessment may be made using site boundary dose rates as defined by the accident classification defined by the monitor reading. Additional, more detailed dose calculations must be performed as soon as possible using EPIP-EOF-5.

(3) Update the above information as frequently as necessary.

4. Report all unexpected emergency results to the Plant Operations blanager for evaluation and recommendations by *.he Technical Support Manager and his staff.
5. If emergency conditions degrade, recommend the Site Area Emergency be re-classified as a General Emergency.
6. Ensure that pertinent operator actions and details of the plant status and changes are recorded in the Operation Log. ])
7. Brief the oncoming shift of all plant conditions and support them if necessary until authorized to leave by the Shif t Operations Supervisor.

EPIP-OSC-6-1 Fort Calhoun Station Unit No. 1 Emergency Plan Implementing Procedure EPIP-0SC-6 ,

GENERAL EMERGENCY ACTIONS I. PURPOPR To outline the steps to be taken by the members of the Operations Staff when a General Emergency is declared.

II. PREREQUISITES A. A General Emergency has been declared on the basis of emergency classification described in EPIP-0SC-1,Section IV.D.

B. The Radiological Emergency Response Plan has been activated as per EPIP-OSC-2.

C. Emergency Procedures EP-1 through EP-37B are available.

III. PRECAUTIONS A. All significant events and action shall be logged in the operation log book.

EPIP-E0F-17-1 Fort Calhoun Station Unit No. 1 Emergency Plan Implementing Procedure EPIP-EOF-17 Initiation of Public Warning I. PURPOSE To outline OPPD's actions for notifying the public of a Site Area or General Emergency.

II. PREREQUISITES A. A Site Area or General Emergency has been declared.

B. Authentication code word for radio station KFAB has been obtained by the Shift Supervisor from the key locker.

III. PRECAUTIONS A. For a Site Area and General Emergency classification each county government must initiate the public warning system within fifteen (15) minutes after notification of the emergency.

B. Be sure that the proper authentication code word for radio station KFAB is used when communicating with them.

C. In all communications, identify yourself to the person contacted and obtain the name and title of the person with whom contact was made.

IV. PROCEDURE

1. Upon declaration of either a Site Area or General Emergency, the Emergency Duty Officer (EDO) or his designee or the Recovery Manager or his designee will notify the county government as listed below and advise them to activate the early warning siren network. The initial report to these agencies, Attachment I to this procedure " Nuclear Power Plant Incident Initial Report to County Agencies", should be made through the dedicated emergency telephone system. As an alternate method for notification, contact the County E0C via the telephone numbers listed below.

2. Request the County EOC to contact radio station KFAB and broadcast the appropriate emergency message. If there is insufficient assurance that KFAB will be contacted, perform steps 3 and 4 of this procedure.

NOTE: Appendix A of this procedure outlines the actions expected to be taken by the county government agencies in completing the public warning process.

3. The EDO or his designee or the Recovery Manager or his designee will notify radio station KFAB to activate the Nebraska Operational Area
  1. 1 Emergency Broadcast Station. Notification to KFAB will be through dial type telephone, (402) 556-5060, using the authentication code word provided by KFAB and available in the Control Room key locker.

4 When contact has been made with radio station KFAB, read the appropriate initial public warning message (s). The public warning messages listed below should be read based on the classification and severity of the accident and the recommended protective action. ! EPIP-EOF-17-10 PUBLIC MESSAGE "Q" SHELTERING In connection with the nuclear power plant incident now underway, State authorities have directed people in certain designated areas near the plant to take shelter immediately. Here are some specific sheltering instructions.

1. If you are outdoors, go inside immediately. Once indoors, close all windows and doors. Turn off fans, air conditioning, and close any other air intakes.
2. If you have come in from outside, wash your hands and face as a minimum, particularly before handling or eating any food. If possible, take a shower using cool or lukewarm water. Wash any items of clothing you were wearing outside.
3. Cover all "open" food containers.
4. Do not use your telephone unless it is absolutely necessary. Keep phone lines open for emergencies.
5. Stay sheltered until you receive official notice that it is safe to go
out. Stay tuned to your emergency broadcast station for later information and further instructions.

APPENDIX A County Government Initiation of Public Warning I. PURPOSE To outline the actions to be taken by county governments after notification by OPPD that a Site Area or General Emergency has been declared at the Fort Calhoun Station Nuclear Power Plant.

II. ' PREREQUISITE A. Communications Officer or responsible person has been given authority to initiate the public warning system.

B. Notification has been made by OPPD that a Site Area or General Emer-

] gency has been declared.

I C. The early warning siren network activation has been advised by the OPPD representative making notification.

III. PRECAUTIONS A. If contact by Fort Calhoun Station is not made through the dedicated emergency telephone system, verify the call by calling back to the Fort Calhoun Station Control Room - (402) 426-4011 or (402) 426-3281.

For a Site Area or General Emergency classification each county B.

EPIP-EOF-18-1 Fort Calhoun Station Unit No. 1 Emergency Plan Implementing Procedure EPIP-EOF-18 0FFSITE RADIOLOGICAL SURVEYS I. PURPOSE To describe the actions to be taken by the offsite monitor team, Emergency Team Tag #5, 6, 7 and 8, in order to locate and define the radioactive plume.

II. PREREQUISITES A. Individuals serving on the team have been trained in offsite radiological monitoring techniques, duties and responsibilities.

B. An " Alert" or greater emergency classification has been declared.

C. Portable radiological survey instruments have been properly checked for operability prior to leaving the site and kept in service through-out monitoring process. g D. Communications equipment has been tested prior to leaving the site.

E. A copy of the " Emergency Radiological Monitor Station Booklet" ic

F. The equipment / supplies listed below are in the monitor vehicle prior to departure from site:

E. When using the radio for communications speak slowly and clearly. (If communications should fail, one member of the team should call the Monitor Coordinator ((402)426-4152 or 426-4276) or return to the E0F for additional communications equipment.)

F. Maintain instruments in service during transit in order to identify trends.

G. Use Channel #1 to communicate with the Radio Operator at the Emergency t

Operations Facility unless instructed otherwise by the Monitor Coordinator.

. H. Count all samples in low background area. This may necessitate leaving the plume area.

IV. PROCEDURE i

1. Assemble at the Emergency Operations Facility (EOF).
2. One member of the team must pickup the keys to the designated emergency monitor vehicle. "'

(NOTE: Keys are located behind the assignment tag in the EOF.) -l

3. Place offsite emergency kit and additional equipment / supplies as listed under II.F of this procedure) into the monitor vehicle which is parked on the East side of the EOF.
4. Energize power to all instruments requiring an external power supply (e.i. SAM-2 and air sampler), as outlined in EPIP-EOF-3,Section IV.3.6.

5. Obtain initial instructions from the Monitor Coordinator for sample type and location for first monitor station. .
6. Locate monitor station using the " Emergency Radiological Monitor Station i

Booklet"; and proceed to the monitor station as described in the directions of the booklet.

7. Begin surveying for radiation dose rate prior to entering vicinity of monitor station. Determine dose rates with either an E-120 with G-M probe or a R0-4 Ion Chamber meter. Record location (s) and dose rate '

, reading (s) into log, Figure 18.1 of this procedure.

8. Report significant dose rate changes to the Monitor Coordinator through the Radio Operator.
9. Coordinate locating the plume's length, width and intensity with the Monitor Coordinator.

10. Prepare SAM-2 for counting iodine cartridge samples, as outlined in EPIP-EOF-3,Section IV.2.c.
11. Setup E-120 with HP-190 probe for counting particulate air samples and smears, as outlined in EPIP-EOF-3,Section IV.2.a.
12. Pull and analyze air samples as directed:
a. Collect the radioiodine sample using a Radeco air sampler in the manner described in EPIP-EOF-3,Section IV.2.c.(2).
b. Analyze the radioiodine sample using the SAM-2 analyzer, in accordance with EPIP-EOF-3,Section IV.2.c.(3).
c. Collect the particulate air sample using a Staplex air sampler, in accordance with EPIP-EOF-3,Section IV.3.a.
d. Analyze the particulate air sample using the E-120 with HP-190 probe, in accordance with EPIP-EOF-3,Section IV.2.a.
13. Smear flat surfaces of 100 cm2 area for each smear.
14. Analyze smears using E-120 with HP-190 probe, as outlined in EPIP-E0F-3,Section IV.2.a.
15. Log all results. (Samples of survey logs 'are provided in Figures 18.1, 18.2 and 18.3.)
16. Report all traults to the Monitor Coordinator through the Radio Operator.
17. Continue surveying for radiation dose rate trends.
18. Proceed to next sample station (s) in accordance with Monitor Coordinator i directions. Identify insturment fluctuations while travelling. At next monitor station, repeat procedure Steps 12 through 17.
19. Continue performing radiological surveys (i.e. air samples, smears, l dose rate surveys) until the full plume is defined and the team is I instructed to return to the EOF.

I l 20. Deliver all samples and logs to the Monitor Coordinator after returning i to the EOF. .
