Incorrectly Ordered Throttling Valves AECM-81/149

Hunt, of your office, of a Potentially Report.able Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerned incorrectly ordered throttling valves for the Residual Heat Removal (RHR)

System, and the Low Pressure Core Spray (LPCS) System. As a result of additional investigative efforts on the part of the Constructor, the Reactor Water Cleanup (RWCU) System and the Fuel Pool Cooling and Cleanup (FPCC)

System have also been shown to be involved.

To date, the full extent of the problem is not certain. Investigative l

efforts are continuing. This report was due on 4/22/81, but on that date an extension until 4/29/81 was granted to Mr. Art Ramey of our office by Mr. F.

S. Cantrell of your office. The reason for the extension request was to confer with our Constructor during a scheduled meeting on 4/23/81. At that meeting we requested that a systematic review take place to ensure that all valves (other than the ones previously identified) specified by design as throttling valves had been ordered correctly. Our Constructor has indicated that the additional review will take some time to complete. Management, Corrective Action Request (MCAR) 99 is the document issued by the Consu;ructor to track this deficiency. g

I. Description of the Deficiency l 1

l Valves for the Low Pressure Core Spray (LPCS) System (E21), the Residual I Heat Removal (RER) System (E12), the Reactor Water Cleanup (RWCU) System l (G33) and the Fuel Pool Cleanup (FPCC) System (G41) were incorrectly purchased as shut-off valves. However, these valves are required to '

operate as throttling valves in order to perform their normal operational safety functions.

This deficiency applies to both Unit 1 and Unit 2.

Reportability per 10CFR50.55(e) was provided on October 13, 1980 by AECM-80/253.

II. Approach to Resolution of the Problem The deficiency is due to incorrec,tly ordered oc'.or operated throttling valves ordered per specification '9645-M-242.0 as shutoff valves. It is believed that all valves affected have been identified; however, the investigation is continuing to confirm this position.: The Constructor has effected coordination with the supplier to evaluate the performance characteristics of each valve in question. In some cases, modification is currently in progress.

Concurrently, the Constructor has issued revisions to the purchase specification to identify those valves requiring a throttling capability to perform the operational function. These revisions will have the effect of precluding recarrence of the problem in regard to Unit 2 construction.

III. Status of Proposed Resolution Modification of the valves by the supplier is underway although no anticipated date of completion of these actions is available at this time.

IV. Reason Why A Final Report Will Be Delayed Verification from the Constructor is being sought to obtain assurance that all affected systems are scrutinized for adherence to design specification. In addition, the problem or "cause" is being addressed to the Constructor to report circumstances that created this deficiency and to establish procedures to preclude such a recurrence.
