- 5. Identification of Occurrence:

On September 21, 1978, a desig: problem with Nuclear Valve Division (Borg Warner) 4 inch, 1500 lb , Carbon Steel Cate Valves was identified.

Five (5) of these valves are ins *alled in the plant. Two (2) valves are spares. Tag numbers for the installed valves are: 2EFW-11A,

2EFW-11B, 2EFW-5B, 2SGS-14A, and 2SGS-14B.

7. Description of Occurrence:

l The problem is a potential for mechanical interference between the valve gate and the gate guide which could prevent complete closure of these manual valves.

9. Analysis of Occurrence:

The problem was discovered during preoperational tests. Since ANO-2 has not been critical there is not a rcdiation hazard nor decay heat to be removed. The health and safety of the general public was not endangered.

10. Corrective Action:

The valves have been returned to the manufacturer for repair.

Corrective action appears to consist of either building up the valve gate or the gate guide at the point of interference to preclude binding.
