Dear Mr. Merschoff:
l Enclosed are applications for renewal of operating license (Form 398) for each of the individuals listed below.
NAME TYPE OF LICENSE Kenneth Wayne Canitz Unit 1 SRO Aaron James South Unit i SRO During the term of their license, these individuals performed their duties in a safe and competent manner and have satisfactorily completed the required requalification.
We require the renewal of these license to meet the commitment in the FS AR and NUREG-0737 for normal shift requirements, retraining, vacation relief, illness and unexpected turnover.
Mr. Canitz is a Simulator Instructor for Arkansas Nuclear One - Unit 1 Mr. South is currently assigned as a classroom instructor for Unit 1.
Also enclosed is a Medical Certification (Form 396) for the above individuals We request 1
