50 382 Waterford Steam Electric Station, Operating License: NPF-38 Unit 3

a During an NRC inspection conducted on October 16-20, 1989, a violation of NRC requirements was identified. The violation involved a failure to take adequate corrective action with regard to following radiation protection procedures.
In accordance with the ' General Statement of Polic and Procedure for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1989) y(Enforcement Policy), the violation is listed below:
Failure to follow Radiological Protection Control procedures Technical Specification 6.11.1 requires, in part, that approved procedures for personnel radiation protection shall be adhered to for all operations involving personnel radiation exposure.
Paragraph 4.8.6 of approved radiation protection Procedure HP-1-110, Fevision 8. " Radiation Work Permits," requires observing and abiding by radiation work permits and posted radiological signs.
Standing Radiation work Permit (RWP) 89000002 requires full ant 1 contamination protective clothing when climbing in a contaminated area.
Contrary to the above, on October 18, 1989, an auxiliary operator entere<1 the Safety injection Pump Room B and proceeded to climb in areas posted as contaminated without wearing the required full anticontamination protective clothing.
This is a repeat of a violation that occurred on May 26, 1989.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201 Louisiana Power & Light Company is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted.
(2) the corrective steps which have been taken and the results achieved.
(3) the corrective steps which will be taken to avoid further violations, and (4)thedatewhenfullcompliancewillbeachieved. Where good cause is shown, consideration will be given to extending the response time.
