Conditions Prior to Event:

The unit was in Mode 5.

Description of Event:

On 7/16/79, at the conclusion of surveillance testing of hydrculic snubbers, a total of 135 snubbers were found not to meet s urveillance test requiren.ents.

Tech. Spec. 3.7.9.1 requires all hydraulic snubbers listed in Table 3.7-4 t c- b e operable. Tech. Spec. 3.7.9.1 action statement requirements were met. During the performance of snubber testing, errors were discovered in Table 3.7-4. a T'ese errors were caused by improper revie:. of the list prior to issuance of the operatins license and typo errors in the final listing included in the Tech. Specs., which was issued by the NRC with the operating lic?ns2. Such errors included erroneous designations of single or doubie anubbers, erroneous snubber numbers and snubbers omitted fran the list. The design organiz2 tion has verified that the above errors do not canstitute design ieficiencies. Due to errors in the table, "arious snubbers were not inspected per Surveillance Requirements 4.7.o.l.b. A re.ised Tech. Spec., Table 3.7-4 will be submitted for apuroval.

Desi2 nation of Apparent Cause:

Sn2bber failures were due to:

1. Bleed rate lid not meet test speci fi cations (S7 snubbers).

2. Lockup crc,blems (21 snubberi).

3. Oil leakage (') snubbers).

4. Blown seals (5 snubbers).

3. Defective fitt tnj;s (2 snubbers).

6. Combination of above failures (11 snubbers).

Also, the reservoir for snubber RC-R219 (located on the pressurizer) has melted.

Analvsis or Event:

The snubber failures we.e discovered while the plant was in Mode 5 and 6.

Ar- a result, the health and safety of the public was not af fected by this oC.urrence.

Corrective Action-u of 7/15/79, 2__ .2:ecti;e snubbers had been repaired and satisfactorily retested. Rerervcir for RC-R219 tras relocated to avoid future reservoir t emperature problems .

A complete inspectica of all snubbers against the revised draft Tech.

Spec. list has been conducted to verify the accuracy of the list. This inspection included cold snubber rod position. Snubbers for which preoperational hot rod position data was ot obtained, will be inspected at hot conditions prior to entering Mode 1.
