The due date for the surveillance had been calculated incorrectly when the surveillance schedules were updated in June 1989. The surveillance had not been performed because of the incorrect due date. The surveillance had actually been due on 09-24-89 and the maximum allevable extension of the surveillance interval was 10-17-89.

This event was caused by cognitive personnel error. The individual who had updated the surveillance schedule in June 1989 improperly scheduled the surveill-ance for a time later than its actual due date.

The actions required by Technical Specification 4.0.3 vere performed. This event has been discussed with the individual who incorrectly updated the surveillance schedule. Personnel responsible for updating the surveillance schedule have been i re-instructed concerning the importance of accurate scheduling of surveillance tests.

Also, a program has been established to provide an independent check of )

the updated surveillance schedule.

r Summary of Event At approximately 1300 on 12-28-89, it was discovered that the required surveillance for plant vent stack radioactive gas monitor RE-14 [IL] had not been performed.

The due date for the surveillance had been calculated incorrectly when the surveill-ance schedules were updeted in June 1989. The surveillance had not been performed because of the incorrect due date. The surveillance had actually_been due on 9-24-89 and the maximum allowable extension of the surveillance interval was 10-17-89.

Description of Event i At approximately 1300 on 12-28-89, the individual who was performing the monthly update of the surveillance schedules discovered that the due date for FNP-2-STP-227.4B (Vent Stack Gas Monitor N2D11RE0014 Functional Test) had-been miscalculated when the schedule was updated in June 1989. FNP-2-STP-227.4B had not been performed because of the scheduling error.

The due date for FNP-2-STP-227.4B was erroneously calculated as if the procedure was required to be performed once per 18 months. The previous due date for the surveillance was 6-24-89. Thus, the due date was calculated to be 12-24-90.- Since the procedure was actually required to be performed quarterly, the correct due date ,

vas 9-24-89 and the maximum allowable extension of the surveillance interval was 10-17-89.

Upon discovery, the actions required by Technical Specification 4.0.3 were '

performed. RE-14 met the requirements of the surveillance procedure. The surveillance was completed at 1446 on 12-28-89.

Cause of Event This event was caused by cognitive personnel error. The individual who had updated the surveillance schedule in June 1989 improperly scheduled FNP-2-STP-227.4B for a time later than its actual due date.

Although the surveillance on RE-14 was approximately 72 days overdue, the radiation monitor met the requirements of the surveillance procedure proving that it had remained operable during this period.- There was no effect on plant operation. The l

health and safety of the public were not affected by this event.  !

Co_rrective Action This event has been discussed with the individual who incorrectly updated the surveillance schedule. Personnel responsible for updating the surveillance.  ;

No component failures occurred during this event.

The unit was available for power operation during the period from 10-18-89 through 12-28-89.

This event vould not have been more severe if it had occurred under different operating conditions.
