ANNUAL ASSESSMENT LETTER FOR GRAND GULF NUCLEAR STATION (REPORT 05000416/2018006)

Dear Mr. Larson:

The U.S. Nuclear Regulatory Commission (NRC) has completed its end-of-cycle performance assessment of Grand Gulf Nuclear Station, reviewing performance indicators (PIs), inspection results, and enforcement actions from January 1, 2018, through December 31, 2018. This letter informs you of the NRCs assessment of your facility during this period and its plans for future inspections at your facility. The NRC concluded that overall performance at your facility preserved public health and safety.

The NRC determined the performance at Grand Gulf Nuclear Station during the most recent quarter was within the Regulatory Response Column, the second highest performance column of the NRCs Reactor Oversight Process (ROP) Action Matrix. Grand Gulf Nuclear Station began the 2018 assessment period in the Regulatory Response Column of the ROP Action Matrix as a result of a White Parallel PI Finding that was effective as of the first quarter of 2017.

This finding was closed based on the results of a supplemental inspection conducted in June 2018, which resulted in the NRC updating its assessment of Grand Gulf Nuclear Station to be in the Licensee Response Column, the highest performance category of the ROP Action Matrix, effective August 2018. Based on a White PI for Unplanned Power Changes per 7,000 Critical Hours (Initiating Events Cornerstone) that was reported in October 2018 for the third quarter of 2018, the NRC updated its assessment of Grand Gulf Nuclear Station to be in the Regulatory Response Column as of the third quarter of 2018. Based on a White PI for Unplanned Scrams per 7,000 Critical Hours (Initiating Events Cornerstone) that was reported in January 2019 for the fourth quarter of 2018, the NRC determined that the performance at Grand Gulf Nuclear Station remained within the Regulatory Response Column.

Therefore, in addition to ROP baseline inspections, the NRC plans to conduct a supplemental inspection in accordance with Inspection Procedure 95001, Supplemental Inspection Response to Action Matrix Column 2 Inputs. The objectives of this procedure are to assure that the root causes and contributing causes of individual and collective significant performance issues are understood, to independently assess and assure that the extent of condition and extent of cause of significant individual and collective performance issues are identified, to assure that corrective actions taken to address and preclude repetition of significant performance issues are prompt and effective, and to assure that corrective action plans direct prompt actions to effectively address and preclude repetition of significant performance issues. This inspection will be scheduled after you notify the NRC of your readiness.

The NRC identified a cross-cutting theme in the Human Performance area. Specifically, 20 findings had a cross-cutting aspect in this area during the 2018 end-of-cycle assessment period. Additionally, two aspects within this area, H.12 Avoid Complacency and H.13 Consistent Process, were each associated with five findings during this assessment period.

The NRC will continue to monitor your staffs effort and progress in addressing this theme until the theme criterion is no longer met.

The enclosed inspection plan lists the inspections scheduled through December 31, 2020.

This updated inspection plan now includes planned security inspections which were formerly transmitted under separate correspondence. The NRC provides the inspection plan to allow for the resolution of any scheduling conflicts and personnel availability issues. Routine inspections performed by resident inspectors are not included in the inspection plan. You should be aware that the agency has proposed changes regarding engineering inspections (SECY-18-0113, Recommendations for Modifying the Reactor Oversight Process Engineering Inspections) and is drafting proposed changes to the ROP for Commission consideration. Should the Commission approve changes to the ROP, the engineering and other region-based inspections are subject to change in scope, as well as schedule, beginning in January 2020. The inspections listed during the last 12 months of the inspection plan are tentative and may be revised. The NRC will contact you as soon as possible to discuss changes to the inspection plan should circumstances warrant any changes.

From December 9, 2016, through February 7, 2019, the NRC issued 10 Severity Level IV traditional enforcement violations associated with impeding the regulatory process. The NRC plans to conduct Inspection Procedure 92723, Follow Up Inspection for Three or More Severity Level IV Traditional Enforcement Violations in the Same Area in a 12-Month Period, to follow up on these violations. This inspection has been scheduled for the week of April 1, 2019.
