'AGE During this period, (November 2 through November 8,.1987), four persons

-were on shift: Paul Geddes, Westinghouse; M.G. Altermatt from the St.

Lucie Plant; P.W. Hughes, Turkey Point Health Physics Supervisor; and J.

A. Labarraque, Technical Department Supervisor.

While on shift, these individuals reported on potential safety problems, questionable work practices, operating strengths, areas for improvement

.and general recommendations.

Four areas of concern were identified during the week:

On November 4, 1987, during the performance of a routine surveillance procedure for the control room emergency ventilation system, three con-duit openings were discovered in the emergency ventiliation fan room which,,

were identified as possible fire barrier breaches. These conduit openings were promptly sealed with metal caps A review of this event to determine the root cause and'proper corrective actions.

2) On November 4, 1987 a concern was identified in that the continuous fire watch in. the cable spreading room was inadequate . When questioned by

'"the'Management'On Shift:"'(MOS) personnel on his duties, the person on watch could not state the complete reason he was stationed in the room.

Additional guidance is being provided to fire watch personnel.

3) During the performance of a routine surveillance procedure for the control room ventilation system, a reactor control operator (RCO) was observed climbing steam pipes to verify the local position of a damper.

The RCO was observed not following Industrial Safety Rules during this evolution. This evolution will be reviewed by the plant Safety Coordinator.

4) On November 5, 1987, a component cooling water (CCW) leak on the seal water heat exchanger for the 3A residual heat removal (RHR) pump was dis-covered during an operability test. The test was being performed to return the second RHR loop to service . The pump passed the operability require-ments of the testing procedures The leak was inspected and it was decided that the leak did not represent an operability problem with the unit in mode 5 and would be fixed when the 3A RHR pump is taken out of service for the recirculation line modification. The pump was then declared back in service.

During the week, several operating strengths were reaffirmed: shift briefings, shift turnover and general procedure compliance. One area of concern identified was inadequate communications involving the status of gobs during shift turnover. Jobs were being reported as completed when in fact they were still being worked. In order to emphasize accurate reporting of )ob status, weekly meeting schedules are being altered to allow increased department head attendance at daily planning meetings .

Questionable work practices At approximately 1:30 A.M., the PSN and the outage (midshift) coordinator discovered 3A steam generator feedwater bypass valve actuator was disassembled without a clearance. Further investigation revealed solenoid valves 3-479A and 3-479B were disconnected without their associated fuses

D. Actions taken PSN took immediate action, determined the original clearance of FCV 479A was temporarily lifted but evidently did not get rehung, PSN hung clearance for FCV-3-479A and fuses for SV-3-479 A and B.

...solenoid valves. (Solenoid valves were on the airline to FCV-479)

D. Actions taken Above was discovered by the PSN. He discovered that the air shutoff valve had had a temporary liftdone on it, but had never been rehung. The temporary liftwas removed, danger tag rehung on the air shutoff valve, and DC power fuses to the solenoid valves were pulIed and tagged OOS. Further investigation revealed that the original scope of work to be done on the valve had been expanded without the clearance being expanded.

1. Unit 4 containment, 58 ft. elevation, a spare control rod drive cooler motor was wired to a seismic support.

2. Hard hats are not being worn inside containments. A plant policy letter dated November 6, 1986 allows this practice. However, I seem to

- remember:this policy was changed. I was unable to find any additional Jetters.

Actions taken

1. ASPN took immediate action and contacted Electrical Department to relocate motor.
2. I willcontact Safety Department to determine correct policy for hard hat usage in containment. Unable to do while on backshift.

., Strengths Work area inside containment for PORV repair was exceptionally neat and organized. The appropriate procedure was on the job site, appropriate tooling was available, etc.

. 3.. I did a complete containment walkdown, except high radiation areas. I was very pleased with system integrity. I found only one valve that showed indication of packing leak.

Reviewed 3-OSP-030.3 "Component Cooling Water System Flowpath Verification" p'rocedure. Acceptance criteria were met.

Observed a possible fire barrier breach, conducted investigation with on-shik PSN,,with

..-'details. explained, in. Section B. A possible, questionable work-practice;arose~from J3us investigation as explained in Section C.

B. Immediate safety problems In the course of observing O-OSP-25.1 being performed, exposed conduit openings were found in the Emergency Ventilation Fan Room, in which were possible fire barrier breaches. Notifying the PSN, an investigation followed in which the PSN decided to call the Bechtel Fire Supervisor and the FPL Fire Supervisor for clarifications. It was found that one open ended conduit was a halon suppression system breach between the Cable Spreading Room inverter area and the Emergency Fan Room. The other 2 questionable conduits extended 3'3" above the floor fire barriers with the conduit extending down into the 4B Electrical Bus Room into an enclosed box.

The box door was latched closed but not securely Iastened. It was questionable that these 2 conduits were fire breaches because of different interpretations of Procedure ASP-31 "Breaching of Fire Protection Systems and Fire Rated Assemblies" and the FSAR, Section 3.15.1, 3.15.2 and

. 3.15.3, Page 9.6 A-107.

The openings were promptly sealed with metal caps by the Bechtel Fire Supervisor. The investigation revealed no administrative control over the work that produced these possible fire barrier breaches.

It is recommended that the Fire Protection Department review its fire barrier breach control records and conduct a review of all known construction work that possibly could produce a fire barrier breach.

MOS DAILYREPORT C. Questionable work practices When conducting O-OSP-25.1 the RCO had to walk a steam pipe holding on to another pipe to verify local position indication of damper D-2. This pipe is 3 stories above the ground with no means of stopping a fall if he slips.

Besides being very dangerous, numerous violations of the FPL Safety Practices rule book were observed. Recommend another means of verifying damper D-2 position be implemented or provide a more safer means of reaching damper D-2 for position verification.

The Security Force Continuous Fire Watch that was stationed in the Cable Spreading Room was asked why he was there. He could only answer that he was a Fire Watch, and could not say he was there because of the loss of the fire detection system and the loss of the halon system which were out for

.maintenance. It appeared from my discussions with him that no briefing was given to him. He was told to find the chair and sit there.

Recommend that the Security Fire Supervisor give briefings to the people under his control and explain the reasons for his/her posting as Fire Watch and review plant procedures on his/her actions on discovering a fire.

C..: . Questionable work practices 3A RHR pump was valved into the RCS in order for Maintenance to check for leaks. Because the plant was in solid plant pressure control, the operator had been instructed to valve it in slowly to prevent depressurizing the RCS. He did not do,it slowly enough, and RCS pressure went low. Seal hP for the 3A RCP fell below 200 psid, and the RCP was tripped. Pressure was returned to normal, start conditions for the RCP were met, and 3A RCP was restarted.

1) 3A RHR pump was declared operable with (what I considered) a significant CCW leak on the seal water heat exchanger. There was a visible stream of water leaking from the HX, and there was water all

, over the floor.

.2) "'A significant amount of bad information particularly from Maintenance is being passed during shift turnover, Specifically, the Unit 4 head vent flanges were reported installed and ready to go a 7:00 AM on 11/5 and were not actually ready to go until approximately 2:00 A.M. on 11/6. There are other examples.

D. Actions taken The shift had been instructed to run the RHR pumps so that the RCS could be depressurized and drained down. This decision appears to have been made outside the Operations Department.
