Failure to ensure barrier openings were secured prior to removing compensatory measures on August 24,1987,(50-424/87-55-01)

Degradeu vital area barrier on September 10,1987,(50-424/87-55-02)

and September 18,1987,(50-424/87-55-03)

Degraded protected area barrier on August 28,1987,(50-524/67-55-04)
