Herdt, Chief, jfCES Branch Date Signed SUMMARY Inspection on January 16-19, 1979 Areas Inspected This routine, unannounced inspection involved 30 inspector-hours on-site in the areas of seismic pipe hangers, construction deficiency reports, noncom-pliances, unresolved items and project status.

Results Of the five areas inspected, no apparent items of noncompliance or deviations were identified in four areas; one apparent item of noncompliance was found in one area (deficiency-nonconformed pipe hanger KC-2129 inspection - Paragragh 5).

Exit Interview The inspection scope and findings were summarized on January 19, 1979 with those persons indicated in Paragraph 1, above.

The scope and findings of the inspection of seismic pipe hanger, construction deficiency reports, noncompliances, unresolved items and project status were discussed. Unresolved item 369/79-05-01 and deficiency 369/79-05-02 (paragraph 5) were discussed.

Licensee Action on Previous Inspection Findings (Closed) Deficiency 369/78-26-01 and 370/78-12-01, Vendor surveillance deficiency reports.

The inspector reviewed the revisior, of DPC QA Procedure QA-602 as reported in the DPC October 14, 1978 letter to RII.

The procedure now provides for two severities of vendor audit findings, variations and deficiencies. RII has no further questions at tnis time.

(Closed) Deficiency 369/78-26-03 and 370/78-12-02, Audit data trend analysis. As stated in the licensee's October 16, 1978 letter to RII, QA Procedure QA-210 has been revised to require a trend analysis of audit data. RII has no further questions at this time.

(Closed) Deficiency 369/78-26-04 and 370/78-12-04, Audit findings corrective action and verifications.

The inspector discussed these findings with the auditor involved in audit E-77-4.

He verified that instructions in verification of corrective action prior to audit closures had been given and all other action outline in DPC's October 16, 1978 letter to RII had been taken. RII has no further questions at this tim.

-2-(Closed) Unresolved Item 369/78-17-01, Piping routed through stacks of safety-related cable trays.

The inspector reviewed the inspection program for cable tray installations.

A]' approved attachments to cable trays support are now identified on sketches, all other have been or will be removed. RII has no further questions regarding this item.

Unresolved Items Unresolved items are matters about which more information is required to determine whether they are acceptable or may involve noncompliance or deviations.

New unresolved items identified during this inspection are discussed in paragraph 5.

5.

Independent Inspection The inspector reviewed the method of inspection of systems for complete-ness for turnover to the Steam Production Department for testing. This review was centered on the KC mechanical systeme and piping. A partial turnover is made to permit pressure testing of system but all exceptions are listed in accordance with DPC Procedure 5-2, Systems / Structures Verification and Turnover. Listed as exceptions for systems turnover at the present are safety-related pipe hangers.

Several hangers still require engineering evaluation and until completed the final QC inspection can not be started.

At present, there does not appear to be a hanger inspection program that is uniform and will ensure that all aspects of the hanger installation are examined.

The licensee advised the inspector that a program is being developed and will become a part of DPC Procedure M-15, Installed Pipe Support Inspection. The development of the final inspection program for pipe hangers and supports will be reviewed by RII and will be identified as unresolved item 369/79-05-01, Final inspection program for pipe hanger and supports.

A review of nonconforming items reports (Q1-A) relating to pipe hangers was conducted. Three Q1-A's, numbers 4672, 2677 and 5075 were selected for examination in that a subcontractor was involved. It was found that resolution for Q1-A 5075 (hanger KC1053) had been submitted. Physical examination of hangers KC2129 (Q1-A 4672) and KC1053 (Q1-A 5075) revealed that neither hanger now had nonconforming tags on them. Further investi-gation revealed that hanger KC2129 had been signed off by the QC inspector but against a hanger sketch that had been revised twice since Q1-A 4672 had been issued. A list of Q1-A's to be resolved did contain Q1-A 4672 but the resolution had not yet been submitted. This instance appears to be an isolated case and a documentation problem in that the hanger would have been reinspected af ter the corrective action before the Q1-A was completed. This will be identified as a deficiency 369/79-Q5-02, noncon-formed pipe hanger KC2129 inspection. During the inspection, the resolution

-3-to Q1-A's 4672 and 5075 were received, and the acceptance of hanger KC2129 was rejected and a new inspection required.

The corrective action appears adequate; therefore, no response to the deficiency 369/79-05-02 is required. IE-II will continue to follow this situation in line with the unresolved item 369/79-05-01 previously identified in this report.

6.

Licensee Identified Items (LII) 10 CFR 50.55(e)

(Closed) LII 369/'8-34-01 and 370/78-14-01, Safety-related containment isolation valve type e leakage deficiency.

The action taken by DPC as in their August 30, 1978 letter to RII appears adequate.RII has no furthec questions at this time.

(Closed) LII 369/78-34-02 and 370/78-14-02, Valve electric motor operator environmental qualification deficiency.

The inspector has reviewed the actions taken as outlined in DPC's August 28, 1978 letter to RII. The corrective action appears adequate and RII has no further questions at this time.

7.

IE Bulletins (IEB) and Circulars (IEC)

IEB 78-10, Bergen-Patterson Hydraulic Shock Suppressor Accumulator Spring Coils.

DPC's letter to RII dated August 28, 1978 advised that Bergin-Patterson snubbers are not in use or planned for use at this station. RII has no further questions at this time.

8.

Project Status-Units 1 and 2 The following personnel assignments have been made by DPC:
