APPENDIX A NOTICE OF VIOLATION Duke Power Company License No.
CPPR-83 McGuire Nuclear Station Unit 1 Based on the NRC inspection of January 16-19, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspon-dence to you dated December 31, 1974.
A.
As required by Criterion XV of Appendix B to 10 CFR 50, as implemented by commitments set forth in the DPC Topical Report, " Quality Assurance Program-Duke-1-A", Section 17.1.15.2 states in part, "The Construction Department has responsibility for resolution of nonconforming item reports concerning deficiencies in fabrication, erection and construction by Duke personnel. The items are tagged or marked to establish their status as nonconforming... Tags which indicate nonconformance can only be removed af ter approval of the resolution of the nonconformance by Quality Assurance."
Contrary to the above, pipe hangers KC 2129 and KC 1053 had been noncon-formed and tagged. The tags became detached. The sketch for KC 2129 was subsequently revised twice, the hanger modified and the modification accepted by the QC inspections.
However, the original nonconforming report had not been resolved. The absence of the nonconforming tag on hanger KC 2129 caused its nonconforming status to be unknown when the subsequent modifications were made.
This is a deficiency.
