Shortly after the restart of Rancho Seco, two of the eight newly installed Gould PD3200 Emergency Feedwater Initiation and Control (EFIC) steam generator low range level transmitters were observed to be drifting low at a rate in excess of the manufacturer's claimed stability. A replacement transmitter also exhibited the same drifting characteristics as the two failed units.
The three failed units were returned to Statham/Gould for inspection and identification of the failure mechanism. Internal inspection revealed that each unit had a leak c,r the filler fluid at the tip of an electronics pin.
Statham/Gould stated that they had experienced some problems with the manufacturing processes during the time the failed units were built.
Rancho Seco has 32 transmitters of the suspect lot. They are in use as follows:
6 EFIC steam generator low range level transmitters 2 Decay Heat cross-tie flow transmitters 4 Reactor Coolant Hot Leg level transmitters 8 EFIC steam generator wide range level transmitters The Gould PD3200 transmitters are being subjected to an intense monitoring program to identify any further problems.

- l' 013 0F 01 3 Statham/Gould subjected the units to a 900 psig static pressure test. The three units exhibited large zero drifts. Subsequent internal inspections revealed that each unit had a leak of the filler fluid at the tip of an electronics pin. Statham/Gould stated that this was indicative of a header brazing process problem. Statham/Gould stated that they experienced some problems with the header brazir.g process during the spring and summer of 1984.
The failed Rancho Seco units were built during this time and have sequential serial numbers. The suspect production lot is the G9700SU series.
The units which failed did not create a safety hazard because the zero drif t was in the conservative direction. The transmitters did not compromise safety while in service; however, had the EFIC full range level transmitters exhibited
, the same large zero drift, it could lead to flooding of the main steam lines during an accident scenario.
Number and location of Such Comoonents at Rancho Seco Rancho Seco has 32 transmitters from the suspect lot. Nine of the transmitters are in stock, three were returned to Statham/Gould, and 20 are currently in use as follows:
6 EFIC steam generator low range level transmitters 2 Decay Heat cross-tie flow transmitters 4 Reactor Coolant Hot Leg level transmitters 8 EFIC steam generator wide range level transmitters One EFIC low range level transmitter and one EFIC wide range level transmitter appear to be exhibiting a similar drift problem at a very low rate; however, this has not been clearly determined.
Rancho Seco has an additional 29 Gould transmitters of a similar type which were not manufactured with the suspect lot. These transmitters have not I exhibited any unusual characteristics. l Corrective Action The three failed units were removed from service and returned to the !
manufacturer for analysis. The remaining Statham/Gould PD3200 transmitters were identified by serial number, service and location. The PD3200 transmitters are being subjected to an intense monitoring program. An Engineering Action Plan has been formulated to monitor the PD3200 transmitters and replace any defective transmitters identified. l l
