The pertinent information required by the regulation is included as attachment No. 1.

This company no longer exists. Spare parts for the Auxiliary Feed Pumps are purchased from Hayward - Tyler, 4.
NATURE OF DEFECT:
The device supplied with the auxiliary feedwater pumps to determine the oil level for the bearings is not a reliable indicator.
The device called th M orthole Oil Level Gauge" is designed with a metal insert behind the sight glass that can cause the oil to be trapped so that a false level of oil is indicated.

Two places on each pump.
- 7. ' CORRECTIVE ACTION:
The interim corrective action is to remove the metal insert. The long term corrective action is to design and install an oil level sight-glass.
